高血压病患者心-踝血管指数与左心室收缩功能的相关性研究

摘　　要：目的探讨高血压病患者心-踝血管指数(CAVI)与左心室收缩功能的相关性及其影响因素。方法纳入213例行CAVI检测并具有血脂、血糖、血浆N末端脑钠肽原(NT-proBNP)、心脏超声等指标的高血压病患者为研究对象,根据CAVI是否升高分为异常组(CAVI≥9.0,n=139)及正常组(CAVI <9.0,n=74)。结果异常组中年龄和NT-pro BNP显著高于正常组,BMI、TC、LDL-C、GFR、Apo B和Apo E显著低于正常组(均P <0.05),左心室射血分数(LVEF)、舒张末期室间隔厚度(IVSD)、舒张末期左室后壁厚度(LVPWd)差异无统计学意义;多元Logistic回归分析显示仅年龄是高血压病患者CAVI增高的的独立危险因素(OR=1.083,P <0.01)。结论在高血压病无明显心力衰竭的患者中,CAVI的增高尚未受NT-pro BNP的影响,但随着年龄的增加,动脉弹性越差。Objective To investigate the correlation between cardio-ankle vascular index(CAVI) arterial stiffness and N-terminnal probrain natriuretic peptide(NT-proBNP) in patients with hypertension. Method 213 patients with hypertension were enrolled,who had data of CAVI, blood lipid, blood glucose, plasma NT-proBNP, and echocardiography. According to the CAVI level, they were divided into abnormal group(CAVI ≥ 9.0, n=139) and normal group(CAVI < 9.0, n=74). Results The age and NTpro BNP in the abnormal group were significantly higher than those in the normal group, the BMI, TC, LDL-C, GFR, ApoB and ApoE in abnormal group were significantly lower than those in the normal group(all P < 0.05). LVEF, IVSD and LVPWd were no significant difference between the two groups. Multivariate Logistic regression analysis showed that age was the risk factor of high CAVI in patients with hypertension(OR=1.083, P < 0.01). Conclusion In hypertensive patients without heart failure, the increase of CAVI is not affected by NT-proBNP, and only age is a risk factor of high CAVI.
