男性与女性肺动脉高压患者心电图特征与肺血管阻力、平均肺动脉压力和心脏指数的关系研究

摘　　要：目的:探讨男性与女性原发性肺动脉高压(PH)患者心电图特征及与肺血管阻力(PVR)、平均肺动脉压力(mPAP)和心脏指数的关系。方法:回顾性分析2018年1月至2020年1月就诊于我院105例原发性PH患者临床资料,按性别不同分为两组,男55例,女50例。分析不同性别PH患者的心电图特征和右心导管结果,并分析心电图与PVR、mPAP、心脏指数的关系。结果:男性组心电图QRS时限、V5导联S波幅高于女性组,差异有统计学意义(P<0.05);在PH患者的心电图中,QRS波的电轴和V1导联的R波振幅与PH患者的PVR呈正相关关系,V1导联的S波振幅与PH患者的PVR呈负相关(P<0.05),V1导联的R波幅与男性PH患者的PVR呈正相关(P<0.05),V5导联的S波幅与女性PH患者的PVR呈正相关(P<0.05);V1导联的R波幅与PH患者的mPAP呈正相关,V1导联的S波幅与PH患者的mPAP呈负相关(P<0.05);心电图中的QRS电轴、V1导联的R/S与男性PH患者的mPAP呈正相关,V5导联的S波幅与女性PH患者的mPAP呈正相关(P<0.05);心电图中V1导联的S波幅与PH患者的心脏指数呈正相关,QT间期与男性PH患者的心脏指数呈正相关(P<0.05)。结论:不同性别PH患者的心电图QRS时限、V5导联的S波幅存在一定差异,且不同性别患者的心电图与PVR、mPAP和心脏指数的相关性存在一定差异。Objective:To investigate the electrocardiogram characteristics of male and female patients with primary pulmonary hypertension(PH)and its relationship with pulmonary vascular resistance(PVR),mean pulmonary artery pressure(mPAP)and cardiac index.Methods:The clinical data of 105 patients with primary PH who were treated in the hospital from January 2018 to January 2020 were retrospectively analyzed.All the patients were divided into male group(55 cases)and female group(50 cases)according to the gender.The electrocardiogram characteristics of PH patients with different genders and the results of right heart catheterization were analyzed,and the relationship of electrocardiogram with PVR,mPAP and cardiac index was analyzed.Results:The QRS duration and S-wave amplitude of V5-lead of electrocardiogram in male group were higher than those in female group,with statistically significant difference(P<0.05);In the electrocardiogram of PH patients,the electric axis of QRS wave and the amplitude of R wave in V1-lead were positively correlated with PVR in PH patients,and the amplitude of S wave in V1-lead was negatively correlated with PVR in PH patients(P<0.05),the amplitude of R wave in V1-lead was positively correlated with PVR of male PH patients(P<0.05),and the amplitude S wave of V5-lead was positively correlated with PVR of female PH patients(P<0.05);The amplitude of R wave in V1-lead was positively correlated with mPAP of PH patients,and the amplitude of S wave in V1-lead was negatively correlated with mPAP of PH patients(P<0.05);The QRS electric axis and R/S in V1-lead on the electrocardiogram were positively correlated with mPAP in male PH patients,and the amplitude of S wave in V5-lead was positively correlated with mPAP in female PH patients(P<0.05);The amplitude of S wave in V1-lead on the electrocardiogram was positively correlated with cardiac index of PH patients,and the QT interval was positively correlated with cardiac index of male PH patients(P<0.05).Conclusions:There are some differences in the QRS duratio
