摘　　要：目的分析结核感染T细胞斑点试验在疑似结核感染的应用价值。方法选取我院2018年1月到2019年7月收治的60例疑似结核感染患者作为研究对象,全部患者均接受T细胞斑点试验和结核菌素试验,将细菌培养结果作为金标准,比较两种试验的诊断准确率、灵敏度和特异度。根据最终诊断结果,比较T细胞斑点试验检测结核病和非结核病的情况。结果T细胞斑点试验的准确度为86.67%(52/60),灵敏度为92.73%(51/55),特异度为80.00%(4/5)。结核菌素试验的准确度为75.00%(45/60),灵敏度为80.77%(42/52),特异度为62.50%(5/8),T细胞斑点试验均高于结核菌素试验,差异有统计学意义(P<0.05)。结核患者T细胞斑点试验诊断准确率为84.62%(33/39),灵敏度为94.12%(32/34),特异度为80.00%(4/5);非结核患者T细胞斑点试验诊断准确率为71.43%(14/21),灵敏度为82.35%(14/17),特异度为75.00%(3/4),结核和非结核诊断相比较,差异无统计学意义(P>0.05)。结论结核感染T细胞斑点试验的应用,在诊断检查疑似结核感染中具有较高的准确率、灵敏度和特异度,值得临床广泛推广及应用。Objective to analyze the application value of t-cell spot test in suspected tuberculosis infection.Methods 60 patients with suspected tuberculosis infection admitted to our hospital from January 2018 to July 2019 were selected as research objects.All patients received t-cell spot test and tuberculin test.Bacterial culture results were taken as the gold standard to compare the diagnostic accuracy,sensitivity and specificity of the two tests.Based on the final diagnosis,the T cell spot test was compared to detect tuberculosis and non-tuberculosis.Results the accuracy,sensitivity and specificity of T cell spot test were 86.67%(52/60),92.73%(51/55)and 80.00%(4/5).The accuracy of the tuberculin test was 75.00%(45/60),the sensitivity was 80.77%(42/52),and the specificity was 62.50%(5/8).The t-cell spot test was higher than the tuberculin test,and the difference was statistically significant(P<0.05).The diagnostic accuracy,sensitivity and specificity of T cell spot test in TB patients were 84.62%(33/39),94.12%(32/34)and 80.00%(4/5).The diagnostic accuracy,sensitivity and specificity of T cell spot test in non-tb patients were 71.43%(14/21),82.35%(14/17)and 75.00%(3/4),respectively.There was no statistically significant difference between TB and non-tb diagnosis(P>0.05).Conclusion the application of T cell spot test for tuberculosis infection has high accuracy,sensitivity and specificity in the diagnosis and examination of suspected tuberculosis infection.
