妊娠早期血清抑制素A、β-HCG与甲状腺功能相关性分析

摘　　要：目的:通过检测妊娠早期血清抑制素A(INH A)、血清人绒毛膜促性腺激素(β-HCG)、血清促甲状腺激素(TSH)、游离甲状腺激素(FT4)及甲状腺过氧化物酶抗体(TPOAB),分析指标间的相关性,探讨其对妊娠期甲状腺功能减退筛查的临床指导意义。方法:选择2018年5月-12月在苏州大学附属第一医院产科产检的8~10周的113例健康孕妇为研究对象,根据血清TSH水平,分为TSH≤4mIU/L组和TSH>4mIU/L组;测定血清抑制素A,β-HCG,TSH、FT4和TPOAB。结果:(1)TSH≤4mIU/L组血清INH A和β-HCG均低于TSH>4mIU/L组,血清FT4高于TSH>4mIU/L组,差异有统计学意义(P<0.05);(2)TSH≤4mIU/L组TPOAB阳性孕妇例数低于TSH>4mIU/L组,差异有统计学意义(P<0.05);(3)逐步回归分析发现血清INH A、β-HCG和TPOAB与TSH之间存在独立相关性。结论:妊娠早期,血清抑制素A和β-HCG与甲状腺功能有关,为妊娠早期甲状腺功能减退的筛查提供临床指导意义。Objective:By detecting the early weeks of pregnancy serum inhibin A(INH A)and human chorionic gonadotropin(beta-HCG),serum thyroid stimulating hormone(TSH),free thyroid hormones(FT4)and thyroid peroxidase antibody(TPOAB),to analyze its relevance to TSH and explore the hypothyroidism during pregnancy screening clinical guiding significance.Methods:113 healthy pregnant women who were admitted to the outpatient department of obstetrics of the First Affiliated Hospital of Soochow University from May to December 2018 for 8 to 10 weeks were selected as the research objects of study.According to the serum TSH level,they were divided into the TSH less than 4mIU/L group and the TSH>4mIU/L group.determined Serum inhibin A,beta-HCG and TSH、FT4 and TPOAB.Results:(1)Serum INH A and beta-HCG in group TSH≤mIU/L were lower than those in group TSH>4mIU/L,and serum FT4 was higher than those in group TSH>4mIU/L,with statistically significant difference(P<0.05).(2)TSH≤4mIU/L group,TPOAB positive pregnant women were lower than TSH>4mIU/L group,the difference was statistically significant(P<0.05).(3)Through multiple stepwise regression analysis,the results showed that serum INH A,beta-HCG,and TPOAB were independent correlation factors of TSH.Conclusion:In early pregnancy,serum statin A and beta HCG are associated with thyroid function,providing clinical significance for screening for hypothyroidism in early pregnancy.
