儿童癫痫主要照顾者院外护理服务需求的质性研究

摘　　要：目的:了解儿童癫痫主要照顾者院外居家照护过程中的护理服务需求,为医护人员实施有效的延续性护理提供依据。方法:采用一对一的深入访谈法,访谈11名照顾者,根据Colaizzi现象学7步分析法对访谈资料进行整理、分析。结果:经过分析得出出院后照顾者有“信息需求”“健康生活指导需求”“心理需求”和“社会支持需求”4个主题,而信息需求又分为癫痫知识信息需求、药物管理信息需求、急性发作应对措施需求3个亚主题。结论:儿童癫痫照顾者出院后仍有迫切的护理服务需求,而信息需求是照顾者居家护理的首要需求,医疗机构应完善儿童癫痫家庭延续性护理服务模式,给予持续性、个体化信息和健康指导,关注照顾者的心理情感体验,使其能正确面对癫痫和应对癫痫发作,提高疾病管理能力,改善家庭生活质量。Objective:To understand the needs of nursing services in the process of out-of-hosptial home care for the main caregivers of children with epilepsy,so as to provide basis for the implementation of effective extended care.Methods:A one-to-one in-depth interview was conducted with 11 caregivers.The interview data were collated and analyzed according to the Seven-step analysis method of Colaizzi.Results:After analysis,it was concluded that there were four themes:information needs,healthy life guidance needs,psychological needs and social support needs.And the information needs were divided into three sub-themes:epilepsy knowledge and information needs,drug management information needs,and acute seizure response needs.Conclusion:The caregivers for children with epilepsy after discharge will still be urgent for care service demand,and information demand was the primary demand for caregivers of home care.Medical institutions should improve family extended care service model for children with epilepsy,and give the persistent and individual information and health guidance,focusing on psychological emotional experience of caregivers.It can make them face epilepsy and epileptic seizures right,improve the ability of disease management,and promote the quality of family life.
