双套管引流治疗非高流量肠外瘘患者的作用

摘　　要：目的研究双套管引流联合早期正常口服饮食在非高流量肠外瘘患者中的治疗作用。方法选择医院2018年6月1日-2019年6月1日收治的非高流量肠外瘘患者200例,随机分为对照组96例和试验组104例,对照组予以双套管引流联合肠内营养制剂治疗,试验组予以双套管引流联合早期正常口服饮食治疗。观察两组患者瘘口自愈率和自愈时间,相关炎症指标水平,包括肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、C-反应蛋白(CRP)。结果两组患者瘘口自愈率差异无统计学意义(χ^2=0.103,P>0.05);试验组自愈时间长于对照组,差异有统计学意义(t=2.540,P<0.05);两组患者治疗后,TNF-α、IL-6、CRP水平均减低,且试验组明显低于对照组,差异有统计学意义(P<0.05)。结论双套管引流联合早期正常口服饮食,可有效减轻非高流量肠外瘘患者的炎症反应,有效的治疗瘘口。Objective To study the therapeutic effect of double cannula drainage combined with early normal oral diet in patients with non-high flow external intestinal fistula.Methods From June 1,2018 to June 1,2019,200 patients with non-high flow external intestinal fistula were randomly divided into control group(96 cases)and experimental group(104 cases).The control group was treated with double cannula drainage combined with enteral nutrition preparation,and the experimental group was treated with double cannula drainage combined with early normal oral diet.The self-healing rate and time of fistulas and the level of related inflammatory indexes were observed,including tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),C-reactive protein(CRP).Results There was no significant difference in the self-healing rate of fistula between the two groups(χ^2=0.103,P>0.05);the self-healing time of the experimental group was longer than that of the control group,and the difference was statistically significant(t=2.540,P<0.05);after treatment,the levels of TNF-α,IL-6,CRP in the two groups were decreased,and the difference was statistically significant(P<0.05).Conclusion double cannula drainage combined with early normal oral diet can effectively reduce the inflammatory response of patients with non-high flow external intestinal fistula,and effectively treat the fistula.
