两种内膜准备方案在冻融胚胎移植的结局分析

摘　　要：目的比较自然周期和激素替代周期两组内膜准备在冻融胚胎移植中的临床结局。方法观察分析2019年1月-2019年12月在医院生殖中心接受冻融胚胎移植的不孕不育患者共450个周期,其中自然周期组(A组)218例,激素替代周期组(B组)232例,比较分析两组患者的子宫内膜厚度、临床妊娠率及早期流产率等情况。结果两组患者年龄、不孕年限、移植胚胎数、移植囊胚比例、转化日子宫内膜厚度等差异均无统计学意义(P>0.05);A组、B组的临床妊娠率、胚胎着床率、早期流产率等差异无统计学意义(P>0.05),但激素替代组临床妊娠率、胚胎着床率略高,与激素替代组年龄偏年轻、移植囊胚比例偏大等有关。结论在冻融胚胎移植中自然周期组与激素替代周期组准备内膜均可获得较好的临床结局,可根据病情、工作安排等选择合适的内膜准备方案。Objective To compare the clinical outcomes of endometrial preparation in freeze-thaw embryo transfer between two groups:natural cycle and hormone replacement cycle.Methods Observation analysis between January 2019 and December 2019 in our hospital reproductive center freezing and thawing embryo transfer cycles of A total of 450 patients with infertility,including natural cycle group 218 cases(group A),hormone replacement cycle group(group B)232 cases,the comparative analysis of two groups of patients with endometrial thickness and clinical pregnancy rate and early abortion rate,and so on and so forth.Results There was no significant difference between the two groups in age,infertility years,number of transferred embryos,proportion of transferred blastocyst and thickness of intrauterine membrane(P>0.05).The clinical pregnancy rate,embryo implantation rate and early abortion rate of group A and group B showed no statistical difference(P>0.05),but the clinical pregnancy rate and embryo implantation rate of hormone replacement group were slightly higher,which was related to the younger age of hormone replacement group and the larger proportion of transplanted blastocyst.Conclusion In freezing-thawing embryo transfer,both the natural cycle group and the hormone replacement cycle can achieve a better clinical outcome,and the appropriate intimal preparation scheme can be selected according to the patient's condition and work arrangement.
