摘 要:目的研究血清成纤维细胞生长因子21(FGF-21)和抗凋亡多肽Humanin(anti-apoptotic polypeptide Humanin,HN)与冠心病(coronary heart disease,CHD)病理进程的关系。方法回顾性分析2017年2月-2019年2月间于本院94例CHD患者(观察组)和47例无冠脉狭窄患者(对照组)临床资料,比较两组患者血清FGF-21与HN水平。记录稳定性心绞痛(stable angina pectoris,SAP)、不稳定性心绞痛(unstable angina pectoris,UAP)及急性心肌梗死(acute myocardial infarction,AMI)三组例数。分析血清GFG-21、HN与冠心病发生发展病理进程的关系。结果观察组患者血清FGF-21显著高于对照组,HN显著低于对照组,差异均有统计学意义(P<0.05)。94例CHD患者中SAP 40例,UAP 28例,AMI 26例。不同病理分型CHD患者HN水平差异有统计学意义(P<0.05)。Pearson相关性分析显示血清FGF-21与Gensini具有良好的相关性(r=0.353,P=0.000)。ROC分析结果显示血清FGF-21判断CHD的AUC为0.899(S.E.=0.029,95%CI=0.841~0.956,P=0.000),灵敏度为0.809,特异度为0.851,最佳截断值为308.5 pg/mL。ROC分析结果显示HN判断UAP或AMI的AUC为0.787(S.E.=0.051,95%CI=0.687~0.887,P=0.000),灵敏度0.815,特异度为0.75,最佳截断值为1.995 ng/mL。结论检测血清FGF-21有助于CHD早期筛查和冠脉狭窄程度的判断,而HN监测有助于预测心血管事件的发生,为临床提供参考。Objective To study the correlation between serum fibroblast growth factor-21(FGF-21)and anti-apoptotic polypeptide Humanin(HN)and the pathological process of coronary heart disease(CHD).Methods The clinical dataes of 94 CHD patients(observation group)and 47 patients without coronary stenosis(control group)from February 2017 to February 2019 in our hospital were retrospectively analyzed,the serum levels of FGF-21 and HN in two groups were compared.The number of patients with stable angina pectoris(SAP),unstable angina pectoris(UAP)and acute myocardial infarction(AMI)were recorded.The relationship between serum GFG-21,HN and the pathogenesis of coronary heart disease were analyzed.Results The serum FGF-21 in the observation group was significantly higher than that in the control group,and HN was significantly lower than that in the control group,and the differences were statistically significant(P<0.05).Of the 94 patients with CHD,40 were SAP,28 were UAP,and 26 were AMI.HN levels in patients with different pathological types of CHD were statistically significant(P<0.05).Pearson correlation analysis showed that serum FGF-21 had a good correlation with Gensini(r=0.353,P=0.000).The results of ROC analysis showed that the AUC of serum FGF-21 to judge CHD was 0.899(SE=0.029,95%CI=0.841 to 0.956,P=0.000),the sensitivity was 0.809,the specificity was 0.851,and the optimal cutoff value was 308.5 pg/mL.ROC analysis results showed that HN judged UAP or AMI to have an AUC of 0.787(S.E.=0.051,95%CI=0.687 to 0.887,P=0.000),a sensitivity of 0.815,a specificity of 0.75,and an optimal cutoff value of 1.995 ng/mL.Conclusion The detection of serum FGF-21 is helpful for early screening of CHD and judging the degree of coronary stenosis,while the HN monitoring is helpful for predicting the occurrence of cardiovascular events and providing reference for clinical practice.
