摘　　要：目的:调查我院血液净化室维持性血液透析(MHD)患者的现状,指导透析质量的持续改进。方法:选择我院2019年5月1日~5月31日既往透析龄≥3月的328例MHD患者为研究对象,采用横断面调查的方法,对其人口学资料、原发病因、透析龄、血管通路、生化指标等临床资料进行分析。结果:328例患者中男166例,女162例,平均57.47±14.38岁。原发病中糖尿病肾病占16.77%。透析龄<5年占65.24%。自体动静脉内瘘使用占80.49%。血红蛋白、铁蛋白、血清白蛋白、校正血钙、血磷、甲状旁腺激素、尿素下降率、单室尿素清除率和透析间期体重增加率的达标率分别为81.10%、29.88%、88.41%、73.48%、48.17%、45.43%,79.27%、82.93%和89.02%。无透析期感染性疾病发生。医保覆盖率为100%。结论:本中心MHD患者以中老年居多,大多透析龄<5年,自体动静脉内瘘使用占比高,透析质控指标达标率较好。Objective:To investigate the current status of maintenance hemodialysis(MHD)patients in our hospital,so as to promote standardization of clinical treatment and continuous quality improvement.Methods:There were 328 patients with MHD more than 3 months from May 1,2019 to May 31,2019 in our hemodialysis center selected for this study.The clinical data were analyzed by cross-sectional survey,including demographic data,primary causes,dialysis age,vascular access and biochemical results.Results:There were 166 males and 162 females,with an average age of(57.47±14.38)years old.Among the definite primary diseases,diabetic nephropathy were accounted for 16.77%.In addition,the dialysis age less than 5 years and autogenous arteriovenous fistula(AVF)were accounted for 65.24%and 80.49%among these 328 cases,respectively.The standard-reaching rate of hemoglobin,ferritin,serum albumin,corrected serum calcium,serum phosphorus,intact parathyroid hormone,urea reduction rate,single-pool urea clearance rate and interdialytic weight gain rate were 81.10%,29.88%,88.41%,73.48%,48.17%,45.43%,79.27%,82.93%,89.02%,respectively.No infectious diseases occurred during the dialysis period.Medical insurance coverage reached 100%.Conclusion:The majority of MHD patients in our center are middle-aged and elderly patients with dialysis less than 5 years,using AVF as vascular access.The standard-reaching rate of dialysis quality indicators in our center have reached domestic advanced level.
