摘　　要：目的探究肠道病毒71型(EV71)感染患儿IL-18水平,分析IL-18基因rs1946518位点多态性与肠道病毒71型感染易感性。方法选择2018年1月-2019年3月浙江大学附属第一医院消化内科收治的EV71感染手足口病患儿80例作为EV71感染组,根据病情严重程度,将患儿分为轻症组46例和重症组34例,选取同期于浙江大学附属第一医院消化内科行体检的健康儿童30名作为对照组,采用ELISA法检测血清IL-18水平,采用散射比浊法检测血清IgG、IgA、IgM及补体C3,C4含量。应用SNaPshot技术检测IL-18基因rs1946518位点多态性。结果EV71感染组的rs1946518位点AA、AC、CC基因型频率分别为28.75%、48.75%、22.50%,与对照组比较差异显著(P<0.05)。重症组的AA基因型频率高于轻症组及对照组(P=0.035、P=0.032);轻症组的CC基因频率与对照组无明显差异,但高于重症组(P<0.05);EV71感染组的rs1946518位点A等位基因频率53.13%、C等位基因频率46.87%,重症组A等位基因频率显著高于轻症组及对照组(P=0.012、P=0.05);EV71感染组的血清IL-18水平显著高于对照组(P<0.05);重症组的血清IL-18水平显著高于轻症组(P<0.05);EV71感染组的血清IgA、IgG水平显著低于对照组,血清IgM、C3、C4水平显著高于对照组(P<0.05);重症组的血清IgA、IgG水平显著低于对照组,血清IgM、C3、C4水平显著高于轻症组(P<0.05)。结论手足口病严重程度与机体的免疫水平有关,IL-18可能参与病情的发生发展过程。IL-18基因rs1946518位点多态性可能与肠道病毒71型易感性有关,A基因可能为易感基因,C基因为保护基因。OBJECTIVE To explore the IL-18 level of the children with enterovirus 71(EV71)infection and observe the polymorphism of IL-18 gene rs1946518 locus and the susceptibility to EV71 infection.METHODS From Jan 2018 to Mar 2019,totally 80 children with EV71-infected hand-foot-mouth disease who were treated in the First Affiliated Hospital of Zhejiang University were chosen as the EV71 infection group and divided into the mild group with 46 cases and the severe group with 34 cases according to the severity of disease,meanwhile,30 healthy children who received physical examination in gastroenterology department of the First Affiliated Hospital of Zhejiang University were set as the control group.The serum IL-18 level was determined by means of ELISA,the contents of serum IgG,IgA,IgM,and complement C3 and C4 were measured by the turbidimetric method,and the polymorphism of rs1946518 of IL-18 gene was detected by using SNaPshot technology.RESULTS The frequencies of AA,AC and CC genotypes in rs1946518 locus of the EV71 infection group were 28.75%,48.75%and 22.50%,respectively,and there were significant differences between the EV71 infection group and the control group(P<0.05);the frequency of AA genotype of the severe group was significantly higher than that of the mild group and the control group(P=0.035,P=0.032);there was no significant difference in the frequency of CC genotype between the mild group and the control group,while the frequency of CC genotype of the mild group was higher than that of the severe group(P<0.05).The frequency of A allele of rs1946518 locus of the EV71 infection group was 53.13%,the frequency of C allele 46.87%;the frequency of A allele of the severe group was significantly higher than that of the mild group and the control group(P=0.012,P=0.05).The serum IL-8 level of the EV 71 infection group was significantly higher than that of the control group(P<0.05);the serum IL-8 level of the severe group was significantly higher than that of the mild group(P<0.05).The levels of serum IgA and IgG of the
