摘 要:目的探讨维生素D水平与早产感染患儿炎性因子水平的关系,分析维生素D水平在预测早产儿感染的临床价值。方法选取2013年1月-2018年12月安徽医科大学第一附属医院收治的86例感染性疾病早产患儿为感染组,选取同期未发生感染的86例早产儿为对照组。比较两组早产儿血清25羟维生素D[25(OH)D]、炎性因子降钙素原(PCT)和C-反应蛋白(CRP)水平,Spearman相关分析感染组患儿25(OH)D水平与血清炎性因子的相关性,探讨25(OH)D水平对预测早产儿感染的临床价值。结果两组早产儿及其母亲一般资料比较差异无统计学意义。与对照组比较,感染组25(OH)D水平显著降低[(17.45±4.92)vs(9.92±2.52)ng/ml],差异有统计学意义(t=12.633,P<0.001);与对照组比较,感染组PCT水平[(0.08±0.02)vs(1.44±0.21)mg/L]、CRP[(3.22±0.54)vs(14.98±3.37)mg/L]水平均显著升高,差异均有统计学意义(t=59.787,31.954;P<0.001)。感染组25(OH)D水平与PCT、CRP水平均呈负相关(r=-0.517,-0.228;P<0.001,0.024)。ROC曲线结果显示,25(OH)D水平预测早产儿感染曲线下面积为0.848 [95%CI(0.784,0.912),P<0.001],临界值为10.53 ng/ml。结论早产感染患儿25(OH)D水平有所降低,并与炎性因子水平呈负相关,且25(OH)D水平对预测早产儿感染具有一定的临床价值。OBJECTIVE To explore the correlation between vitamin D and inflammatory factors of premature infants with infection and analyze the clinical value of vitamin D in prediction of infection in the premature infants.METHODS Totally 86 premature infants with infectious diseases who were treated in First Affiliated Hospital of Anhui Medical University from Jan 2013 to Dec 2018 were chosen as the infection group,meanwhile,86 premature infants without infection were set as the control group.The levels of serum 25-hydroxyvitamin D[25(OH)D],inflammatory factor procalcitonin(PCT)and C-reactive protein(CRP)were compared between the two groups of premature infants,Spearman correlation analysis was performed for the correlation between the25(OH)D and the serum inflammatory factors of the premature infants in the infection group,and the clinical value of the 25(OH)D in prediction of infection in the premature infants was explored.RESULTS There was no significant difference in the baseline data between the two groups of premature infants and between the two groups of premature infants′mothers.The 25(OH)D level of the infection group was(9.92±2.52)ng/ml,significantly lower than(17.45±4.92)ng/ml of the control group(t=12.633,P<0.001).The levels of PCT and CRP of the infection group were respectively(1.44±0.21)mg/and(14.98±3.37)mg/L,significantly higher than(0.08±0.02)mg/L and(3.22±0.54)mg/L of the control group(t=59.787,31.954;P<0.001).The 25(OH)D level was negatively correlated with the levels of PCT and CRP(r=-0.517,-0.228;P<0.001,0.024).ROC curve showed that the area under curve of the 25(OH)D was 0.848[95%CI(0.784,0.912),P<0.001]in prediction of infection in the premature infants,and the cut-off value was 10.53 ng/ml.CONCLUSION The 25(OH)D level of the premature infants with infection is reduced and is negatively correlated with the levels of inflammatory factors,and the 25(OH)D has certain clinical value in prediction of infection in the premature infants.
