摘　　要：目的探讨肺动脉起源的室性心律失常体表心电图特征,为临床诊疗提供依据和参考。方法选择2017年5月~2019年5月起源于肺动脉的室性心律失常患者12例,设为观察组;选择同期治疗的起源于右室流出道肺动脉瓣下的患者48例,设为对照组。两组入院后均给予体表心电图检查,记录患者Ⅱ、Ⅲ、avF导联R波、avL/avR Q波振幅比值、V2导联R/S的比值和胸导联移行区。结果观察组体表心电图中Ⅱ导联R波振幅、Ⅲ导联R波振幅、avF导联R波振幅、avL与avR的Q波振幅的比值、V2导联R/S波振幅比值均高于对照组(P<0.05);观察组胸导联移行区中V2、V3、V5及V6病例比例均多于对照组(P<0.05);观察组胸导联移行区中V4病例比例低于对照组(P<0.05)。结论将体表心电图用于肺动脉起源的室性心律失常患者中能实现肺动脉起源的室性心律失常定位,能为临床诊疗提供依据和参考。Objective To investigate the characteristics of the surface electrocardiogram of ventricular arrhythmia with origin of pulmonary artery,and to provide basis and reference for clinical diagnosis and treatment.Methods 12 patients with ventricular arrhythmia originating from the pulmonary artery from May 2017 to May 2019 were selected as the observation group.48 patients originating from the right ventricle outflow tract pulmonary artery valve during the same period were selected as the control group.After admission,the two groups were given surface electrocardiogram examination.TheⅡ,Ⅲ,avF lead R wave,avL/avR Q wave amplitude ratio,V2 lead R/S ratio and chest lead transition area of patients were recorded.Results The R wave amplitude of leadⅡ,Ⅲand avF,the ratio of Q wave amplitude of avL and avR,and the ratio of R/S wave amplitude of lead V2 were all higher in surface ECG of the observation group than those in the control group(P<0.05).The proportion of V2,V3,V5 and V6 cases in the chest lead transition area of the observation group was more than that in the control group(P<0.05).The proportion of V4 cases in the chest lead transition area of the observation group was lower than that of the control group(P<0.05).Conclusion The use of surface electrocardiogram in patients with ventricular arrhythmia of pulmonary artery origin can locate ventricular arrhythmias with pulmonary artery origin,which can provide basis and reference for clinical diagnosis and treatment.
