空军飞行学员医学选拔定选阶段肾下垂内科触诊与超声检查结果分析

摘　　要：目的探讨招飞医学选拔过程中肾下垂的诊断方法及检查流程,为提高肾下垂诊断准确率和优化肾下垂检查流程提供参考依据。方法收集2019年参加空军飞行学员医学选拔定选的3 290名学员肾脏触诊和超声检查结果,对结果进行分析。结果内科触诊肾下垂阳性102例,检出率为3.1%,均为右肾。内科触诊肾下垂阳性者进一步行超声检查,超声检查判断肾下垂阳性36例,阴性66例。2种检查方法判断肾下垂程度一致性一般,加权Kappa系数为0.267,差异具有统计学意义(P<0.001)。肾脏位置越低,肾下极移动度越大(P<0.05)。结论超声检查判断肾下垂程度的准确性高于内科触诊。Objective To evaluate the diagnostic methods and examination process of nephroptosis in order to provide reference for improving the diagnostic accuracy and optimizing the examination process of nephroptosis.Methods The results of renal palpation and ultrasound were collected from 3290 cadets who participated in the medical selection of Air Force flying cadets in 2019 before they were compared.Results A total of 102 cases of nephroptosis were positive and all of them were related to the right kidney,with a detection rate of 3.1%.The cases with positive nephroptosis were further examined by ultrasound.The results showed that 36 of these cases were positive and 66 were negative.The difference between the two methods was statistically significant,and the weighted kappa coefficient was 0.267(P<0.001).The lower the position of the kidney in case of nephroptosis,the greater the mobility of the lower pole(P<0.05).Conclusion Ultrasound examination can more accurately assess the degree of nephroptosis than internal palpation.
