不稳定性心绞痛患者的血清蛋白质组学研究

摘　　要：目的进行冠状动脉正常人群和不稳定性心绞痛患者的血清蛋白质表达谱检测,寻找与不稳定性心绞痛相关的特异蛋白质,希望能为不稳定性心绞痛的防治提供新靶点。方法以冠状动脉正常人群和不稳定性心绞痛患者的血清为研究对象,应用双向荧光差异凝胶电泳(2DE-DIGE)和质谱分析技术,对不稳定性心绞痛及冠状动脉正常人群各100例,行血清蛋白质组学研究,并选取2个具有代表性的差异蛋白应用ELISA进行验证。结果经2DE-DIGE/MS分析,发现11种19个差异蛋白质点发生了改变,涉及到急性期反应、炎症反应、细胞骨架、脂/能量代谢蛋白。上调的蛋白有:急性期蛋白包括铜篮蛋白、血清淀粉样蛋白A2以及α-1抗胰蛋白酶;补体系统的C4和C6前体。下调的蛋白有:载脂蛋白A-Ⅰ、脂蛋白A-Ⅳ及其前体;转铁蛋白;转甲状腺素蛋白;细胞骨架的凝溶胶蛋白、凝溶胶蛋白-1亚基、Myosin-11。ELISA验证显示急性冠状动脉综合征患者,血清淀粉样蛋白A表达升高,凝溶胶蛋白降低。结论急性期反应蛋白、补体系统及凝血相关蛋白的异常,载脂蛋白、细胞骨架蛋白的降低,能量代谢紊乱等一系列的病理生理改变参与不稳定性心绞痛的进展,凝溶胶蛋白、血清淀粉样蛋白A2等特异蛋白质点有可能成为不稳定性心绞痛防治的新靶点,但尚需进一步研究来证实。Objective To find the special plasma molecules in the plasma protein map from patients with unstable angina using proteomics.Methods The plasma from 200 patients(100 with unstable angina(UA)and 100 with normal coronary angiography)was detected.2-DE-DIGE/MS analysis was performed during the procedure.Results Based on 2DE-DIGE/MS analysis,19 different expression proteins were found in the patients of ACS.It involves acute phase reaction,inflammatory reaction,cytoskeleton,lipid/energymetabolism protein.Serum amyloid A2,CP20 kDa protein,alpha-1 antitrypsin,haptoglobin beta and alpha-2chain,complement component C6 precursor and C4 were up-regulated in the plasma from UA patients while apolipoprotein A-Ⅰ,A-Ⅳand A-Ⅳprecursor,TF 11 and 7kDa protein,transthyretin,gelsolin and gelsolin precursor isoform 1,and Myosin-11 were down-regulated.Moreover,ELISA analysis showed that SAA was up-regulated while gelsolin was down-regulated in the plasma from UA patients.Conclusion Various proteins involving acute phase protein,the complement system,cytoskeleton,apolipoprotein,energy metabolism participated in the procession of UA.The newly discovered different proteins,serum amyloid A2 and gelsolin might be special molecules for UA.But more investigation is needed in the future.
