摘  要:目的探讨艾灸"足三里""梁门"对胃热证、胃寒证模型大鼠胃黏膜病理及脑皮质组织代谢物谱的影响。方法 42只SD大鼠随机分为空白组10只、胃热模型组16只、胃寒模型组16只。胃热模型组采用辣椒液、乙醇灌胃方法复制胃热证大鼠模型,胃寒模型组采用冰水+氢氧化钠(NaOH)灌胃方法复制胃寒证大鼠模型。造模后将各模型组随机抽取6只作为艾灸胃热组、艾灸胃寒组。空白组、胃热模型组、胃寒模型组不予干预,艾灸胃热组、艾灸胃寒组大鼠于"足三里""梁门"穴行温和灸干预,每次20min,每天1次,连续干预7天。HE染色观察胃黏膜组织病理学变化;对脑皮质进行代谢组学检测,分析所得数据及代谢通路。结果胃热模型组、胃寒模型组胃黏膜组织符合胃热证、胃寒证的病理表现,艾灸胃热组、艾灸胃寒组大鼠胃黏膜组织病理较模型组改善。代谢组学发现12个与胃热证密切相关、11个与胃寒证密切相关的脑皮质代谢物。艾灸能调节胃热证9个、胃寒组5个代谢物的紊乱。艾灸主要调节脑皮质的牛磺酸-亚牛磺酸代谢、烟酸-烟酰胺代谢、磷酸肌醇代谢,丙氨酸-天冬氨酸-谷氨酸代谢对胃热证胃黏膜损伤起修复作用;经缬氨酸-亮氨酸-异亮氨酸的生物合成,磷酸肌醇代谢对胃寒证胃黏膜损伤起修复作用。结论艾灸对胃热证胃黏膜修复的主要效应机制涉及经中枢对抗氧化应激、氨基酸代谢和维持细胞形态的调节作用,对胃寒证胃黏膜修复的主要效应机制涉及经中枢对生物体内氨基酸代谢、维持细胞形态调节。Objective To study the effects of "Zusanli(ST 36)" and "Liangmen(ST 21)" on pathology of gastric mucosa and metabolite profile of cerebral cortex in rats with stomach heat syndrome, and stomach cold syndrome. Methods A total of 42 male SD rats were randomly divided into a blank group(n = 10), a stomach heat model group(n = 16), and a stomach cold model group(n = 16). In the model of stomach heat syndrome, the rat model of stomach-heat syndrome was replicated by gavage with chili liquid and ethanol, and the rat model of stomach-cold syndrome was replicated by gavage with ice water and sodium hydroxide(NaOH). After modeling, 6 rats from each model group were randomly selected as moxibustion stomach heat group and moxibustion stomach cold group. The blank group, stomach heat model group and stomach cold model group did not intervene, and the rats in moxibustion stomach heat group and moxibustion stomach cold group underwent mild moxibustion intervention at "Zusanli(ST 36)" and "Liangmen(ST 21)" for 20 min, once daily for 7 days. HE staining was used to observe the histopathological changes of gastric mucosa;metabolomic examination of the cerebral cortex was performed to analyze the data and metabolic pathways. Results The gastric mucosal tissues in the stomach heat model group and the stomach heat model group conformed to the pathological manifestations of the stomach heat syndrome and the stomach cold syndrome. The pathology of the gastric mucosa in the moxibustion stomach heat group and the moxibustion stomach cold group was improved compared with the model group. Metabolomics found that 12 cerebral cortex metabolites were closely related to the stomach heat syndrome and 11 were closely related to the stomach cold syndrome. Moxibustion mainly regulates the metabolism of taurine and hypotaurine, niacinamide and phosphatidylinositol metabolism, and metabolism of alanine aspartic acid and glutamic acid in the cerebral cortex.
