摘 要:目的探究6 min步行试验(6minute walk test,6MWT)在新型冠状病毒冠肺炎(COVID-19)康复期患者功能评估中的应用价值.方法随机选取2020年4~5月就诊于湖北省中医院的87例COVID-19出院患者进行6MWT,运动前、中、后监测并记录患者的心率、血氧饱和度、心电图、呼吸频率,运动前后记录患者血压.统计分析6 min步行试验距离(6 minute walk distance,6MWD)、体重指数(BMI)、血氧平均值、血氧饱和度变化分数、心率平均值、心率升至最大心率值时间、心率变化分数各因素间的相关性,分析不同BMI组各因素与6MWD的相关性.结果6MWD与运动中的血氧饱和度平均值、心率平均值、心率变化分数正相关(P<0.05);血氧饱和度变化分数与心率平均值正相关(P<0.05);心率变化分数与心率升至最大心率时间、血氧饱和度变化分数、6MWD及心率平均值正相关(P<0.05);正常BMI组6MWD与血氧平均值正相关,高BMI组6MWD与心率平均值正相关(P<0.05).结论6MWT操作简单,结果可靠,能够有效地对COVID-19出院患者的心肺功能状态评估,进行患者日常功能状态的监测,建议在临床上作进一步推广.Objective To evaluated the application value of the 6-minute walk test(6MWT)for CO VID-19 rehabiltation.Methods 87 COVID-19 patients are chosen and tested randomly in Hubei Provincial Hospital of TCM from April to May 2020.Their heart rates(HR),blood oxygen saturation(SpO2),ECG and respiration rates(RESP)were recorded before,during and after the tests;the blood pressues(BP)were recorded before and after the tests.The correlations among 6MWD,BMI,the mean values of SpO2,the change values of Sp02,the mean values of HR,the rising times of HR and the change values of HR were analyzed statistically.The correlations between the indexes with 6MWD,among the different BMI groups were also analyzed.Results 6MWD is positively correlated with the mean values of SpO2,the mean values of HR and the change values of HR.The change values of SpO2 are positively correlated with the mean values of HR(P<0.05).The change values of HR were positively correlated with the rising times of HR,the change values of SpO2,6MWD and the mean values of HR(P<0.05).6MWD was positively correlated with the mean values of SpO2 in the normal BMI group(P<0.05).6MWD was postively correlated with the mean values of HR in the high BMI group(P<0.05).Conclusion 6MWT is a simple and reliable method to evaluate the discharged COVID-19 patients'cardiopulmonary functions and can also be used to monitor their daily health status.It is suggested to use 6MWT more widely in clinical cardiopulmonary rehabilitation.
