摘 要:目的探讨三维适形调强放疗(intensity-modulated radiation therapy,IMRT)对股骨继发性骨包虫病鼠腹股沟淋巴结T淋巴细胞的影响及机制。方法股骨继发性骨包虫病子午沙鼠20只,随机分为IMRT组和对照组各10只,IMRT组股骨包虫病灶区给予40 Gy的IMRT放射治疗,对照组给予假照射。放疗结束1周后,采用电镜观察2组鼠骨包虫细胞形态学改变情况,采用免疫荧光法观察鼠腹股沟淋巴结CD4^+、CD8^+T淋巴细胞表达情况,应用流式细胞仪检测鼠腹股沟淋巴结CD4^+、CD8^+T淋巴细胞比率,计算CD4^+/CD8^+。结果放疗1周后,IMRT组鼠股骨包虫细胞大量坏死,对照组鼠股骨包虫细胞形态正常;IMRT组鼠腹股沟淋巴结CD4^+、CD8^+T淋巴细胞大量表达,对照组鼠腹股沟淋巴结CD4^+、CD8^+T淋巴细胞少量表达;IMRT组鼠腹股沟淋巴结CD4^+T淋巴细胞比率[(89.73±12.14)%]和CD4^+/CD8^+(5.29±0.69)高于对照组[(71.16±11.03)%、2.89±0.37],CD8^+T淋巴细胞比率[(16.96±3.14)%]低于对照组[(24.65±4.29)%](P<0.05)。结论IMRT对鼠股骨继发性骨包虫有较好的杀灭效果,其作用机制可能与放疗激发宿主对骨包虫有效的免疫反应有关。Objective To investigate the influence and mechanism of intensity-modulated radiation therapy(IMRT)on T-lymphocyte subsets of inguinal lymph nodes in Meriones meridianuses with secondary femoral bone echinococcosis.Methods Twenty Meriones meridianuses with secondary femoral bone echinococcosis were randomly and equally divided into IMRT group and control group.IMRT group received IMRT at a total dose of 40 Gy,and control group received no IMRT.In 1 week after IMRT,the cellular morphological changes of bone hydatid were observed by electron microscopy,the expressions of CD4^+ and CD8^+ were observed by immunofluorescence,and percentages of CD4^+ and CD8^+ were detected by flow cytometry.Results In 1 week after IMRT,massive necrosis of femoral hydatid was found in IMRT group,while the morphology of femoral hydatid was normal in control group.CD4^+ and CD8^+ were highly expressed in IMRT group,and lowly expressed in control group.The CD4^+ percentage and CD4^+/CD8^+ were higher in IMRT group((89.73±12.14)%,5.29±0.69)than those in control group((71.16±11.03)%,2.89±0.37),and the CD8^+ percentage was lower in IMRT group((16.96±3.14)%)than that in control group((24.65±4.29)%)(P<0.05).Conclusion IMRT can effectively kill secondary femoral bone echinococcosis in Meriones meridianuses,probably by encouraging host effective immune response.
