摘　　要：目的:探讨米非司酮、米索前列醇联合彩超治疗内生型瘢痕妊娠的临床效果。方法:回顾性分析2015年8月—2018年8月我院妇科收治住院的96例内生型瘢痕妊娠患者的一般资料,按照治疗方式分为观察组(58例)与对照组(38例)。观察组采用米非司酮和米索前列醇药物治疗,并在彩超引导下行无痛清宫术;对照组在子宫动脉介入中注入甲氨蝶呤并予栓塞治疗后给予常规清宫治疗。比较两组总住院金额、自费金额、平均住院日、清宫术中出血量、治疗前后HCG下降幅度以及清宫术后HCG恢复正常的天数。结果:观察组的总住院金额、自费金额低于对照组;平均住院日、清宫术中出血量少于对照组、治疗前后HCG下降幅度小于对照组,差异有统计学意义(P<0.05),两组清宫术后HCG恢复正常的天数相近,差异无统计学意义(P>0.05)。结论:米非司酮和米索前列醇联合彩超治疗内生型瘢痕妊娠临床疗效较好,相对安全,且住院时间短、费用低,值得推广。Objective:To explore the clinical effect of mifepristone and misoprostol combined with color ultrasound in the treatment of endogenous scar.Methods:General data of 96 patients with endogenous scar pregnancy admitted and hospitalized in our hospital from August 2015 to August 2018 were retrospectively analyzed,treatment was divided into observation group(58 cases)and control group(38 cases).Patients in the observation group were treated with mifepristone and misoprostol,and performed vacuum aspiration under the guidance of color ultrasound;patients in the control group were injected with methotrexate during uterine artery intervention and performed vacuum aspiration after artery embolization.The total hospitalization amount,self-paying amount,average length of stay,intraoperative bleeding,HCG decrease before and after treatment,and the time when HCG decreased to normal were compared between the two groups.Results:The total hospitalization amount and self-paying amount in the observation group were lower than those in the control group.The average length of stay,the amount of intraoperative bleeding was less than that of the control group,and the decrease of HCG before and after treatment was less than that of the control group,with statistically significant differences(P<0.05).The time when HCG decreased to normal was similar between the two groups,with no statistically significant differences(P>0.05).Conclusion:The combination of mifepristone and misoprostol with color ultrasound in the treatment of endogenous scar pregnancy has a good clinical effect and is relatively safe,and the hospital stay is short and the cost is low,which is worthy of promotion.
