摘 要:目的探讨分析不同风湿免疫性疾病患者的免疫功能差异。方法选取2016年1月至2017年9月大连市中心医院风湿免疫病房收治的类风湿性关节炎(RA)、强直性脊柱炎(AS)、骨关节炎(OA)和痛风性关节炎(AG)患者62例为研究对象,另选取同期25例健康人作为对照组。比较不同风湿免疫性疾病患者外周血T淋巴细胞亚群、Treg细胞数量和PD-1表达的差异。结果与对照组比较,RA、AS、AG组患者CD4^+T细胞明显升高,CD8^+T细胞所有组均明显下降,所有组CD4^+/CD8^+比值与对照组比较均明显升高。发现Treg细胞在每组中变化也不同,与对照组比较,RA组患者Treg细胞没有变化,AS、OA、AG组患者Treg细胞明显升高。还检测了CD4^+T细胞和CD8^+T细胞上PD-1的表达。与对照组比较,AS组和OA组CD4^+T细胞上PD-1表达明显升高,而RA组和AG组无明显变化,而每一组患者CD8^+T细胞上PD-1表达均出现明显降低。结论不同风湿免疫性疾病的免疫病理损伤及组织器官类型不一样,其自身免疫特点也不尽相同,但共同点为免疫功能紊乱,不同的风湿免疫性疾病中T淋巴细胞亚群、Treg和PD-1均存在明显的不同,免疫调节异常是风湿免疫性疾病发生发展的重要因素。Objective Explore and analyze the differences in immune function of patients with different rheumatic immune diseases.Methods In this study,62 patients with reumatoid arthritis(RA),ankylosing spondylitis(AS),osteoarthritis(OA)and gouty arthritis(AG)admitted to the rheumatic immunity ward of Dalian Central Hospital from January 2016 to September 2017 were selected as subjects to compare the difference of T lymphocyte subsets,Another 25 healthy people in the same period were selected as the control group,Treg cell and PD-1 expression in peripheral blood.Results Compared with the control group,CD4^+T cells in RA,AS,AG group were significantly increased,CD8^+T cells in all groups were significantly decreased,and the ratio of CD4^+/CD8^+in all groups was significantly higher than that in control group.We found that the changes of Treg cells in each group were also different.Compared with the control group,Treg cells in RA group did not change,but Treg cells in AS,OA,AG group increased significantly.We also detected the expression of PD-1 on CD4^+T cells and CD8^+T cells.Compared with the control group,the expression of PD-1 on CD4^+T cells in AS and OA group was significantly increased,but there was no significant change in RA and AG group,but the expression of PD-1 on CD8^+T cells in each group was significantly decreased.Conclusion The immuno-pathological injury and types of tissue and organ in different rheumatic immune diseases are different,and their autoimmune characteristics are different,but the common denominator is immune dysfunction.There were significant differences in T lymphocyte subsets,Treg and PD-1 in different rheumatic immune diseases.The abnormal immunomodulation is an important factor in the occurrence and development of rheumatoid immune diseases.
