摘 要:目的分析新生儿化脓性脑膜炎脑脊液(CSF)的特点及病原菌情况。方法选择2015年1月至2017年12月在本科住院治疗的148例化脓性脑膜炎患儿作为研究对象,分析病原学、脑脊液实验室检查结果,根据脑脊液白细胞计数增高程度的不同,对比脑脊液蛋白和葡萄糖的定量,以及治疗后脑脊液细胞计数恢复正常的天数。结果随着脑脊液细胞计数的增高,脑脊液蛋白定量及白细胞计数恢复正常的天数比较差异有统计学意义(P<0.05)。在脑脊液及血液中培养出的病原菌以革兰阴性杆菌多见,其中以大肠埃希菌最多,血培养21.6%(32/148)、脑脊液培养9.5%(14/148)。脑脊液细菌培养阳性率低于血培养。结论脑脊液白细胞计数增高程度影响疗程及病原学培养率,大肠埃希菌是引起该病较常见的病原菌。Objective To analyze the characteristic of cerebrospinal fluid(CSF)and pathogenic bacteria of neonatal purulent meningitis.Methods 148 cases of neonatal purulent meningitis in our hospital from January 2015 to December 2017 were selected as study subjects.The pathogen and laboratory examination of CSF were analyzed.According to the differently increased degree WBC count of CSF,quantitative protein and glucose,the number of days that CSF cell count returned to normal after treatment were compared.Results As the CSF cell count increased,there was statistically significant difference between protein quantification and restore normal WBC count days(P<0.05).The Gram-negative bacteria was more than Gram-positive in both blood and CSF culture.The Escherichia coli was the most common bacteria,with positive rate of 21.6%(32/148)in blood culture and 9.5%(14/148)in CSF culture.The germiculture positive rate in CSF was lower than in blood culture.Conclusion The treatment and culture rate of pathogen were influenced by WBC count increased degree.Escherichia coli was a common bacteria caused by this disease.
