Pre-S1 Ag、Pre-S2 Ag及HBV-LP在乙肝病毒检测中的性能比较

摘　　要：目的通过对乙肝前S1抗原(Pre-S1 Ag)、乙肝前S2抗原(Pre-S2 Ag)及乙肝病毒大蛋白(HBV-LP)三种乙肝表面蛋白表达情况的检测,分析比较其灵敏度和特异性,以期获得适合常规乙肝筛查的血清标志物组合。方法应用ELISA法,分别检测健康人群和乙肝患者血清中三种表面蛋白的表达情况,利用实时荧光定量PCR法检测HBV-DNA载量。结果应用于HBV筛查时,三种表面蛋白特异性分别为97%、100%和100%,但Pre-S2 Ag和HBV-LP对HBsAg携带者、HBV-DNA阳性患者的灵敏度显著高于Pre-S1 Ag(P<0.0001),并且三种乙肝病毒表面蛋白阳性检出率随HBV DNA水平增高而递增,Pre-S2 Ag、HBV-LP对HBV的检测限接近于10^3IU/ml而Pre-S1 Ag约为10^5IU/ml。结论作为乙肝筛查的标志物,Pre-S2 Ag及HBV-LP更优于Pre-S1 Ag。Objective By detecting the expression performance of Pre-S1 Ag,Pre-S2 Ag and HBV-LP,the study compared their sensitivity and specificity to get suitable serum markers combination for routine hepatitis B virus(HBV)screening.Methods The Pre-S1 Ag,Pre-S2 Ag and HBV-LP in healthy people and HBV patients were detect⁃ed by ELISA method.HBV-DNA load was detected by quantitative real-time PCR.Results For the diagnosis of HBV,the specificity of Pre-S1 Ag,Pre-S2 Ag and HBV-LP were 97%,100%and 100%,but the sensitivity of Pre-S2 Ag and HBV-LP were significantly higher than that of Pre-S1 Ag for HBsAg carriers and HBV DNA positive patients(P<0.0001).Their positive detection rate increased by the level of HBV DNA.The detection limit of Pre-S2 Ag and HBVLP to HBV were 10^3 IU/mL,but that of Pre-S1 Ag was about 10^5 IU/mL.Conclusion For HBV screening marker,the Pre-S2 Ag and HBV-LP were better than Pre-S1 Ag.
