摘 要:目的:研究冠心病慢性心力衰竭(CHD+CHF)患者水停证与心脏超声指标间的相关性。方法:选择本院2016年1月~2018年1月间收治的60例CHD+CHF水停证患者作为水停证组,同期收治的60例CHD+CHF非水停证患者作为非停水证组,对比两组NYHA心功能分级、水停证症状积分,心脏超声指标:心脏指数(CI)、左室心肌质量指数(LVMI)、分析水停证症状积分、NYHA分级、心脏超声指标之间的相关性。结果:与非水停证组比较,水停证组NYHA II级比例、LVEF、左室短轴收缩率(LVFS)、CI均显著降低(P均<0.01),NYHAⅣ级比例、左房内径(LAD)、左室收缩末期内径(LVESd)、LVMI、水停证症状积分均显著升高(P均<0.01)。Pearson相关分析显示,水停证症状积分与CI、LVFS、LVEF呈显著负相关(r=-0.707^-0.983,P均=0.001),与LAD、LVESd、LVMI呈显著正相关(r=0.563~0.997,P均=0.001)。Spearman相关分析显示,NYHA心功能分级与LVMI、LAD、LVESd、水停证症状积分呈显著正相关(r=0.691~0.927,P均=0.001),与CI、LVFS、LVEF呈显著负相关(r=-0.691^-0.902,P均=0.001)。结论:随CHD+CHF患者心功能显著下降,水停证病情加重,且CHD+CHF水停证积分与NYHA心功能分级、LVMI、LAD、LVESd呈显著正相关,与CI、LVFS、LVEF呈显著负相关。Objective:To study correlation among water retention syndrome(WRS)and echocardiographic indexes in patients with coronary heart disease(CHD)complicated chronic heart failure(CHF).Methods:A total of 60 CHD+CHF patients with WRS treated in our hospital from Jan 2016 to Jan 2018 was selected as WRS group.Another 60 CHD+CHF patients without WRS were enrolled as non-WRS group simultaneously.New York heart association(NYHA)cardiac function class,WRS symptom score and echocardiographic indexes:cardiac index(CI),left ventricular mass index(LVMI)were compared between two groups.Correlation among WRS symptom score,echocardiographic indexes and NYHA class were analyzed.Results:Compared with non-WRS group,there were significant reductions in percentage of NYHA class II,LVEF,left ventricular shortening fraction(LVFS)and CI(P<0.01 all)and significant rise in percentage of NYHA classⅣ,left atrial diameter(LAD),left ventricular end-systolic dimension(LVESd),LVMI and WRS symptom score in WRS group(P<0.01 all).Pearson correlation analysis indicated that CI,LVFS and LVEF were significant inversely correlated with WRS symptom score(r=-0.707^-0.983,P=0.001 all),and LAD,LVESd and LVMI were significant positively correlated with WRS symptom score(r=0.563~0.997,P=0.001 all).Spearman correlation analysis indicated that NYHA class was significant positively correlated with LVMI,LAD,LVESd and WRS symptom score(r=0.691~0.927,P=0.001 all),and significant inversely correlated with CI,LVFS and LVEF(r=-0.691^-0.902,P=0.001 all).Conclusion:Along with cardiac function decreases,WRS aggravates in CHD+CHF patients;the WRS symptom score is significantly positively correlated with NYHA cardiac function class,LVMI,LAD,LVESd is significantly negatively correlated with CI,LVFS,LVEF.
