摘 要:目的:确定月份、孕期(妊娠早期和妊娠晚期)对妊娠期维生素D(Vit D)的影响效应,以及妊娠期Vit D大体变化趋势。方法:选取2017年4月至2018年3月一年周期内在本院产检和分娩的单胎妊娠孕妇作为研究对象,在妊娠早期行25-羟维生素D[25-(OH)D]水平检测孕妇2839例,在妊娠晚期行25-(OH)D水平检测孕妇1536例,妊娠早期和妊娠晚期同时行25-(OH)D检测孕妇985例。分析1)一年内同月份妊娠早期和妊娠晚期孕妇血25-(OH)D水平情况;2)一年内同一孕妇妊娠早期和妊娠晚期25-(OH)D水平混合线性模型分析。结果:1)一年的周期内,相同月份的妊娠早期和妊娠晚期25-(OH)D水平比较,差异均无统计学意义(P>0.05);2)孕期(妊娠早期和妊娠晚期)对25-(OH)D水平无显著影响(F=3.173,P=0.075),月份对25-(OH)D水平的影响是显著性的(F=37.068,P=0.000)。结论:在妊娠过程中,妊娠期Vit D水平在妊娠早期和妊娠晚期并无显著变化,对妊娠期Vit D水平产生重要影响的是月份,应鼓励妊娠女性在妊娠期间多参加户外活动,进行Vit D补充时,可根据月份调整Vit D的剂量。Objective:To determine the influential effect of month,pregnancy period(early and late pregnancy)on vitamin D and the general trend of vitamin D during pregnancy.Methods:Single pregnant women who received routine prenatal examination and delivered in our hospital during the one-year cycle from April 2017 to March 2018,were selected as research objects,the level of 25-hydroxyvitamin D[25-(OH)D]were tested.There were 2839 pregnant women in the early pregnancy and 1536 pregnant women in the late pregnancy.The 25-(OH)D levels of 985 pregnant women were tested simultaneously in the early and late pregnancy.Analysis:1)the blood levels of 25-(OH)D in pregnant women of the same month in the early and late pregnancy;2)mixed linear model analysis of 25-(OH)D levels in the early and late pregnancy of the same pregnant woman.Results:1)There was no significant difference in 25-(OH)D levels between the early and late pregnancy in the same month(P>0.05).2)Pregnancy period(early and late pregnancy)had no significant effect on 25-(OH)D level(F=3.173,P=0.075),the month had a significant effect on 25-(OH)D level(F=37.068,P=0.000).Conclusion:During pregnancy,vitamin D level has no significant change in the early and late stages of pregnancy,the month has an important impact on vitamin D.Pregnant women should be encouraged to participate in outdoor activities during pregnancy,and the dose of Vitamin D Supplementation can be adjusted according to the month.
