彩色多普勒超声和MSCTA在主动脉夹层中的影像学表现及分型检出情况

摘　　要：目的分析彩色多普勒超声和MSCTA在主动脉夹层(AD)中的影像学表现以及分型检出情况。方法选取2016年4月~2019年4月我院收治的主动脉夹层患者80例,分别采用彩色多普勒超声和MSCTA检测,记录两种方法的影像学表现,比较两种方法AD不同分型检出率及总检出率。结果超声检查影像学表现中特异性征象包括撕裂内膜瓣、真腔、假腔80例,撕裂内膜破裂口50例;非特异性征象包括主动脉扩张80例,附壁血栓64例。MSCTA检查影像学表现中特异性征象包括主动脉腔不同密度真假双腔80例,分离、移位内膜瓣80例,内膜增厚、钙化48例,内膜破裂口63例;非特异性征象包括病变段主动脉扩张80例,主动脉壁增厚、钙化76例,附壁血栓25例。MSCTAT检测AD总检出率为95.00%,高于超声的62.50%,差异有统计学意义(P<0.05),其中超声及MSCTA检测A1、A2、A3、B1、B3型AD检出率比较,差异无统计学意义(P>0.05);超声及MSCTA检测检测B2型AD检出率比较,差异有统计学意义(P<0.05)。结论两种方法的影像学表现不同,MSCTA检测AD分型检出率检出率高于彩色多普勒超声检测。Objective To analyze the imaging findings and type detection of color Doppler ultrasound and MSCTA in aortic dissection(AD).Methods A total of 80 patients with aortic dissection treated in our hospital from April 2016 to April 2019 were selected.Color Doppler ultrasound and MSCTA were used to record the imaging performance of the two methods,and the different types of AD were compared between the two methods.Detection rate and total detection rate.Results The specific signs of ultrasound imaging include 80 cases of tearing the intimal flap,true cavity and false cavity,50 cases of tearing the endometrial rupture;non-specific signs include 80 cases of aortic dilation and 64 cases of mural thrombosis.Specific signs in the imaging findings of MSCTA examination include 80 cases of aortic lumen with different densities,80 cases of separation and displacement of endometrial valve,48 cases of intimal thickening and calcification,and 63 cases of intima rupture;non-specific signs included 80 cases of dilated aorta,76 cases of aortic wall thickening and calcification,and 25 cases of mural thrombosis.The total detection rate of AD by MSCTAT was 95.00%,which was higher than 62.50%of ultrasound,the difference was statistically significant(P<0.05).Among them,the detection rates of AD by A1,A2,A3,B1 and B3 were compared by ultrasound and MSCTA,the difference was not statistically significant(P>0.05);compared with the detection rate of B2 AD by ultrasound and MSCTA,the difference was statistically significant(P<0.05).Conclusion The imaging performance of the two methods is different.The detection rate of AD typing by MSCTA is higher than that by color Doppler ultrasound.
