改良早期预警评分表在新生儿病房病情观察中的应用

摘　　要：目的运用改良新生儿早期预警评分表,识别新生儿病情恶化信号,及时采取相关治疗措施,改善治疗结局。方法选取2016年在郑州大学第三附属医院新生儿病房住院的患儿2563例为研究对象,其中1~6月住院患儿为对照组,7~12月住院患儿为观察组,对比分析两组间患儿病情恶化、转入新生儿重症监护室(NICU)的发生率和治疗结局,比较预警评分表应用前后医生对护士呼叫时机的满意度。结果观察组与对照组患儿病情恶化发生率差异无统计学意义(P>0.05),转入NICU的发生率差异无统计学意义(P>0.05),但治疗结局差异有统计学意义(P<0.05);改良早期预警评分最佳界值为2.5分,其识别新生儿病情恶化灵敏度为84.9%,特异度为86.2%;医生对护士呼叫时机的满意度差异有统计学意义(P<0.05)。结论在新生儿病情观察中应用改良早期预警评分表,可以帮助不同年资护士早期识别病情恶化信号,提高监护质量,改善治疗结局,具有临床推广价值。Objective To explore the application effect of Modified Early Warming Scale(MEWS)in the deterioration of observation of newborns,timely adoption of relevant treatment measures to improve the outcome of treatment.Methods 2563 hospitalized newborns who were born in 2016in the neonatal ward were selected as the observed group object,including the hospitalized children during Jana and Jun of 2016 as control group,and the hospitalized children during July and December as observation group.Comparative analysis of the deteriorationof children's condition between the two groups,and the incidence of shifting to NICU,the treatment outcome and the satisfaction of doctor about when the nurse called them were compared.Results The incidence of disease deterioration between two groups had no statistical significance(P>0.05),the incidence of shifting to NICU between two groups had no statistical significance(P>0.05),but the treatment outcome was statistically significant(P<0.05).The optimal threshold for the improved early warning cut off value was 2.5,The sensitivity and specificity in identifying the deterioration of neonatal disease were 84.9%and 86.2%.The satisfaction of doctor about when the nurse called them was statistically significant(P<0.05).Conclusions MEWS can help nurses with different seniority early recognize the deterioration of the state of illness,improve the quality of care and treatment outcome,therefore,MEWS should be widely used in Neonatal Ward.
