摘 要:群体药代动力学是实现个体化治疗的重要方法,而中医的辨证论治更是典型的个体化治疗,与群体药代动力学理念高度一致。笔者通过试验证实:同一种中药/方剂用于不同证候患者,药物代谢存在差异。已有研究表明:药物代谢差异的重要原因是表观遗传机制。证候与表观遗传均体现环境等非先天因素对机体的影响,表观遗传机制可能是证候的内在本质。因此,推测表观遗传可能是不同证候患者药代动力学差异的机制所在。笔者提出:以表观遗传药理学为切入点,研究不同证候患者药物代谢的差异及其机制,可以成为中药群体药代动力学研究的新思路。Population pharmacokinetics is an important method to achieve individualized treatment,and syndrome differentiation and treatment of traditional Chinese medicine is a typical individualized treatment,which is highly consistent with the concept of population pharmacokinetics.The author confirmed through the experiment:the same traditional Chinese medicine/prescription used in patients with different syndromes,is different in drug metabolism.Previous studies have shown that the important reason for the difference in drug metabolism is epigenetic mechanism.Syndrome and epigenetics both reflect the influence of non-congenital factors such as environment on organism.Epigenetic mechanism may be the intrinsic nature of syndrome.Therefore,it is speculated that epigenetic inheritance may be the mechanism of pharmacokinetics differences in patients with different syndromes.The authors suggest that epigenetic pharmacology as a starting point to study the difference of drug metabolism and its mechanism in patients with different syndromes can be a new way to study the pharmacokinetics of traditional Chinese medicine.
