金硝逐瘀汤对痔疮术后并发症及创面愈合情况的影响

摘　　要：目的观察金硝逐瘀汤对痔疮术后并发症及创面愈合的影响。方法采用随机数字表法将我院82例痔疮术后患者随机分为观察组、对照组各41例,两组均行外剥内扎术治疗,术后均予以常规抗生素预防感染。对照组应用马应龙麝香痔疮膏治疗,观察组在对照组治疗基础上联合应用金硝逐瘀汤治疗。比较两组患者术后1d及治疗后3d、5d疼痛程度,采用视觉模拟评分量表(VAS)评分治疗5d后创缘水肿情况,术后1d、治疗5d后创面肉芽组织与创面面积评分变化,以及血清生化指标[血小板源性生长因子(PDFG)、碱性成纤维生长因子(bFGF)、血管内皮生长因子(VEGF)]检测水平。结果两组患者术后1d VAS评分对比差异无统计学意义(P>0.05),观察组给药1d后、给药3d后、给药5d后VAS评分均显著低于对照组,差异均有统计学意义(均P<0.05);治疗5d后,观察组患者创缘水肿情况优于对照组,差异有统计学意义(P<0.05);两组患者创面肉芽组织、创面面积评分均较术后1d降低,且观察组低于对照组,差异均有统计学意义(均P<0.05);两组患者血清PDFG、bFGF、VEGF均较治疗前升高,且观察组明显高于对照组,差异均有统计学意义(均P<0.05)。结论痔疮术后应用金硝逐瘀汤可有助于缓解术后疼痛、肛缘水肿等并发症,有利于创面愈合。Objective To observe the effects of Jinxiao Zhuyu decoction on postoperative complications and wound healing of hemorrhoids. Methods 82 cases of postoperative patients with hemorrhoids were randomly divided into observation group and control group,41 cases each. Both groups underwent Milligan-Morgan hemorrhoidectomy,and given routine antibiotics after operation to prevent infection,control group was given Mayinglong Shexiang Zhichuang ointments,observation group was combined with Jinxiao Zhuyu decoction on the basis of control group. The pain degree [visual analogue scale(VAS)] score 1 d after operation and 3 d and 5 d after treatment,wound edge edema 5 d after treatment,scores of wound granulation tissue and wound area 1 d after operation and 5 d after treatment and serum biochemical indicators [platelet-derived growth factor(PDFG),basic fibroblast growth factor(bFGF),vascular endothelial growth factor(VEGF)] were compared between the two groups.Results There was no significant difference in the VAS scores between the two groups 1 d after operation(P>0.05),and the VAS scores in observation group were significantly lower than those in control group 1 d,3 d and 5 d after administration(all P<0.05). After 5 d of treatment,the wound edema in observation group was better than that in control group(P<0.05). Thescores of wound granulation tissue and wound area in the two groups were lower than those 1 d after operation,and the scores in observation group were lower than those in control group(all P<0.05). The serum levels of PDFG,bFGF and VEGF in the two groups were higher than those before treatment,and the levels in observation group were significantly higher than those in control group(all P<0.05). Conclusion The application of Jinxiao Zhuyu decoction after hemorrhoids can help relieve postoperative pain,anal edge edema and other complications,and it is conducive to wound healing.
