摘 要:目的对比分析STIR技术和SPAIR技术在心脏磁共振成像中评价心肌水肿方面的优缺点。方法回顾性分析临床确诊病毒性心肌炎且行心脏磁共振检查的患者34例,按心律情况分为A、B两组,所有患者在心肌参数扫描时分别使用STIR技术和SPAIR技术行左心室T2WI黑血序列。通过主观评分及CNR值测量,应用重复测量方差分析及简单效应分析对比两种抑脂技术的检查效能。结果对于心律齐的患者,两种评价方法SPAIR技术所获图像质量均优于STIR技术所获图像,简单效应分析差异有统计学意义(P<0.05);在心律不齐短轴位的检测中,主观评价提示STIR技术得分高于SPAIR技术,长轴位上无差异,CNR评价长轴位和短轴位检测中STIR技术得分均低于SPAIR技术,简单效应分析差异有统计学意义(P<0.05)。结论对于怀疑心肌水肿的患者,为了获得清晰满意的左心室壁的T2WI抑脂序列图像,应优先采用SPAIR技术扫描,STIR技术可作为补充。Objective To compare and analyze the advantages and disadvantages of STIR and SPAIR in the evaluation of cardiac edema in cardiac magnetic resonance imaging.Methods A retrospective analysis of 34 patients with clinically confirmed viral myocarditis and cardiac magnetic resonance examination was performed.All patients used STIR technology and SPAIR technology to perform left ventricular T2WI black blood sequence during myocardial parameter scanning.Through visual score and CNR measurement,data was grouped according to heart rhythm.Repeated measurement analysis of variance and simple effect analysis were used for statistics.Results For the patients with arrhythmia,the quality of the images obtained by the two evaluation methods SPAIR technology was better than that obtained by the STIR technology.The difference of the simple effect analysis was statistically significant,P<0.05.Visual observation showed that the STIR technology score was higher than the SPAIR technology,and there was no difference in the long axis position.The STIR technology scores in the CNR evaluation of the long axis position and the short axis position were lower than the SPAIR technology.The difference in simple effect analysis was statistically significant,P<0.05.Conclusion For patients with suspected myocardial edema,in order to obtain a clear and satisfactory T2WI fat-suppression sequence of the left ventricular wall,the SPAIR technique should be used for scanning,and STIR technique can be used as a supplement.
