活血解郁汤联合针刺四花穴对脑卒中后抑郁症患者血浆5-HT、DA和NE的影响

摘　　要：目的:观察活血解郁汤联合针刺四花穴治疗脑卒中后抑郁症患者的疗效其及对血浆5-HT、DA和NE的影响。方法:选取符合纳入标准的脑卒中后抑郁症患者135例,按照随机数字表随机分为观察组、西药组和对照组,每组45例。观察组采用口服自拟活血解郁汤联合针刺四花穴治疗;西药组给予氟哌噻吨美利曲辛片,每天1片,晨起顿服;对照组只给予安慰剂治疗,4周为1个疗程,共治疗3个疗程。观察两组患者治疗前后的血浆5-HT、DA、NE水平、抑郁程度及生活质量评分的变化情况。结果:3组治疗前血浆5-HT、DA、NE水平、抑郁程度及生活质量评分比较无明显差异,经相应处理后,观察组和西药组受试者血浆5-HT、DA、NE水平均较治疗前及对照组明显增高(均P<0.05),且观察组5-HT、DA水平优于西药组(P<0.05),对照组治疗后无明显差别(P>0.05)。治疗后观察组及西药组患者的HAMD评分均下降,明显优于对照组(P<0.05),且观察组的HAMD评分优于西药组(P<0.05)。观察组及西药组患者的生活质量各项评分均有所提高,且明显高于对照组(P<0.05)。同时,观察组治疗后评分比西药组提高更为明显(P<0.05)。结论:活血解郁汤联合针刺四花穴可明显改善脑卒中后抑郁患者的抑郁状态及日常生活质量,且能明显提高患者血浆5-HT、DA水平。Objective:To observe the effect of Huoxue Jieyu Decoction Combined with needling Sihua points on post-stroke depression.Methods:135 patients with post-stroke depression were randomly divided into the observation group,the western medication group and the control group,with 45 cases in each group.The observation group was treated with Huoxue Jieyu Decoction combined with needling Sihua points,the western medication group was treated with Flupentixol,and the control group was only given placebo.Four weeks was taken as one course of treatment.After three courses of treatment,plasma levels of 5-HT,DA and NE were detected,and depression degree and quality of life were assessed before and after the treatment in the three groups.Results:There were no statistical differences in plasma levels of 5-HT,DA,NE,depression degree and quality of life among the three groups before the treatment.After corresponding intervention,the plasma levels of 5-HT,DA and NE were significantly higher in the observation group and in the western medication group than those in the control group(P<0.05);and the levels of 5-HT and DA were higher in the observation group than those in the western medication group(P<0.05).There was no significant differences in the levels of 5-HT,DA and NE between before and after the treatment in the control group(P>0.05).The HAMD scores significantly decreased in the observation group and in the the western medication group than those in the control group after the treatment(P<0.05);the decreases in the observation group were more significant(P<0.05).The scores of quality of life were better improved in the observation group and in western medication group than those in the control group after the treatment(P<0.05);the improvements in the observation group were more significant(P<0.05).Conclusion:Huoxue Jieyu Decoction combined with needling Sihua points can significantly improve the depression state and life quality of the patients with post-stroke depression,it can significantly improve the plasma levels of 5-HT
