超声引导股神经阻滞麻醉对膝关节镜患者术后快速康复的影响研究

摘　　要：目的探讨超声引导股神经阻滞麻醉是否能促进接受膝关节镜手术患者快速康复。方法选择我院2015年4月~2016年9月择期拟行膝关节镜手术术患者60例,随机分为实验组和对照组,各30例。试验组实施神经阻滞麻醉,对照组实施腰硬联合麻醉。比较两组患者术后疼痛、并发症情况和患者恢复情况。结果实验组VAS评分术后8 h(0.9±0.55)分和术后12 h(1.533±0.63)分低于对照组(t=14.847,13.46,P<0.05),两组患者术后2 h和24 h的VAS评分无统计学意义(P>0.05),实验组患者不良反应发生率3.33%低于对照组43.33%(x2=13.416,P<0.05),实验组恢复功能锻炼时间(10.93±4.23)h及住院时间(6.07±0.87)d均短于对照组(t=22.023,6.391,P<0.05)。结论膝关节镜手术行超声引导股神经阻滞麻醉术后镇痛效果好,并发症少,可促进患者尽快恢复,值得推广应用。Objective To explore whether ultrasound-guided femoral nerve block anesthesia can promote the rapid recovery of patients undergoing knee arthroscopic surgery.Methods 60 patients who were scheduled to undergo knee arthroscopic surgery in our hospital from April 2015 to September 2016 were randomly divided into experimental group and control group,30 cases in each group.The experimental group was anesthetized with nerve block,while the control group was anesthetized with combined spinal-epidural anesthesia.The postoperative pain,complications and recovery were compared between the two groups.Results The VAS score at 8 h(0.9±0.55)and 12 h(1.533±0.63)of experimental group was lower than control group(t=14.847,13.46,P<0.05).There was no significant difference in VAS score between two groups at 2h and 24h after operation(P>0.05).The incidence of adverse reactions in experimental group was 3.33%lower than control group(x2=13.416,P<0.05).The recovery time of functional exercise(10.93±4.23)h and hospitalization time(6.07±0.87)day in experimental group were shorter than those in the control group(t=22.023,6.391,P<0.05).Conclusion Ultrasound-guided femoral nerve block anesthesia for knee arthroscopic surgery has good analgesic effect and fewer complications.It can promote the recovery of patients as soon as possible and is worthy of popularization and application.
