表柔比星膀胱灌注致过敏性休克一例

摘　　要：患者,女性,64岁,退休,以“体检发现左输尿管开口占位2天”入院,积极术前准备,于2018.12.09在全麻下行膀胱肿瘤电切术备膀胱部切加左输尿管再植术,于7.29行表柔比星膀胱灌注,灌注后10分钟左右出现双上肢皮肤搔痒,面部皮肤潮红,15分钟左右,出现过敏反应,即给予高流量吸氧,静脉推注肾上腺素1 mg、15分钟后症状缓解,约1 h后患者病情稳定。此后停用法码新,患者无再类似发作。对接受表柔比星等化疗的患者,用药前应详细询问过敏史,用药过程中注意观察和询问患者有无不适,对过敏体质者应慎用或使用过程加强监护。还要备好抢救药品,以免给患者造成不必要的麻烦和损失,确保用药的安全、有效[1]。The patient,female,64 years old,retired,was admitted to the hospital with"physical examination found that the left ureteral orifice occupied 2 days",and actively prepared before operation.On September,2018,12,he underwent the resection of bladder tumor under general anesthesia for the resection of bladder and the replantation of left ureter,At 7.29,epirubicin was infused into the bladder,and the skin of both upper limbs was scratched about 10 minutes later,the skin of the face was flushed,and the allergic reaction occurred about 15 minutes later.That is to say,high flow oxygen was given,adrenaline was given intravenously for 1 mg,and the symptoms were relieved after 15 minutes,and the patient's condition was stable about 1 hour later.After that,the patient stopped using the new code and had no similar attack.For the patients receiving epirubicin chemotherapy,they should inquire the history of allergy in detail before using the medicine,observe and inquire whether the patients have discomfort during the course of using the medicine,and strengthen the monitoring for the patients with allergic constitution.It is also necessary to prepare rescue drugs to avoid unnecessary troubles and losses to patients,so as to ensure the safety and effectiveness of medication(1)。
