曼月乐与热球子宫内膜去除术对异常子宫出血患者生活质量和性激素水平的影响  

Effects of Manyuele and Hot-bulb Endometrial Ablation on Quality of Life and Sex Hormone Levels in Patients with Abnormal Uterine Bleeding

摘　　要：目的对比异常子宫出血患者采用曼月乐与热球子宫内膜去除术治疗效果及对患者生活质量和性激素水平的影响。方法便利选取该院2017年3月-2019年3月收治的异常子宫出血患者65例,根据治疗方法的不同,将行曼月乐治疗的30例纳入对照组,行热球子宫内膜去除术治疗的35例纳入观察组,对比两组临床疗效、治疗前后PBAC评分、子宫内膜厚度、Hb水平、生活质量评分及性激素水平变化。结果观察组与对照组治疗总有效率相当(97.14%vs 93.33%)(χ^2=1.684,P>0.05);观察组治疗6个月后的PBAC评分、Hb水平、子宫内膜厚度分别为(39.64±5.18)分、(116.83±10.59)g/L、(5.19±1.33)mm,均显著优于对照组的(50.36±5.73)分、(110.30±9.89)g/L、(6.89±1.40)mm(t=7.920、2.555、5.014,P<0.05);观察组治疗6个月后的E2、LH、FSH水平分别为(0.33±0.15)pmol/L、(16.25±2.61)mIU/mL、(13.52±3.10)mIU/mL,与对照组的(0.36±0.12)pmol/L、(16.05±2.72)mIU/mL、(12.83±3.16)mIU/mL相当(t=0.880、0.302、0.887,P>0.05);观察组治疗6个月后的生活质量总评分为(96.78±10.38)分,显著高于对照组的(91.32±9.83)分(t=2.175,P<0.05)。结论对于异常子宫出血患者,采用曼月乐与热球子宫内膜去除术治疗均可获得满意的治疗效果,但热球子宫内膜去除术可显著减少患者PBAC评分,改善患者贫血状况和性激素水平,提高患者生活质量。Objective To compare the treatment effect of Manyuele and hot-bulb endometrial ablation in patients with abnormal uterine bleeding and the effect on patients'quality of life and sex hormone levels.Methods Sixty-five patients with abnormal uterine bleeding treated in the hospital from March 2017 to March 2019 were conveniently selected.According to the different treatment methods,30 patients treated with Manyuele were included in the control group and treated with hot bulb endometrial ablation.Thirty-five cases were included in the observation group,and the clinical efficacy,PBAC score before and after treatment,endometrial thickness,Hb level,quality of life score,and sex hormone levels were compared between the two groups.Results The total effective rate of treatment in the observation group was comparable to that of the control group(97.14%vs 93.33%)(χ^2=1.684,P>0.05);the PBAC score,Hb level,and endometrial thicknessof the observation group after 6 months of treatment were respectively(39.64±5.18)points,(116.83±10.59)g/L,and(5.19±1.33)mm,which were significantly better than the control group's(50.36±5.73)points,(110.30±9.89)g/L,and(6.89±1.40)mm(t=7.920,2.555,5.014,P<0.05);E2,LH,and FSH levels in the observation group after 6 months of treatment were(0.33±0.15)pmol/L and(16.25±2.61)mIU/mL,(13.52±3.10)mIU/mL,equivalent to(0.36±0.12)pmol/L,(16.05±2.72)mIU/mL,(12.83±3.16)mIU/mL of the control group(t=0.880,0.302,0.887,P>0.05);the total score of quality of life in the observation group after 6 months of treatment was(96.78±10.38)points,which was significantly higher than that of the control group(91.32±9.83)points(t=2.175,P<0.05).Conclusion For patients with abnormal uterine bleeding,both Manyuele and hot-bulb endometrial ablation can be used to obtain satisfactory results,but hot-bulb endometrial ablation can significantly reduce the PBAC score of patients,improve their anemia status and sex hormone levels.To improve the quality of life of patients.
