摘　　要：目的探讨艾滋病(AIDS)合并马尔尼菲青霉菌病(PSM)患者的临床特点及腹部CT表现。方法回顾性分析该院2008年1月-2019年6月186例确诊为AIDS合并PSM的患者的临床资料及腹部CT影像资料。结果所有患者CD4+T淋巴细胞均<50个/μL,91.39%(170/186)的患者腹部CT异常,其中腹腔淋巴结肿大135例(72.58%),肝脏肿大110例(59.14%),脾脏肿大128例(68.82%),肝脾均肿大101例(54.30%),脂肪肝85例(45.70%),腹腔积液25例(13.44%),肝脏及脾脏"镂空"征19例(10.22%),肠壁增厚7例(3.76%)。结论 AIDS合并PSM多发生在CD4+T淋巴细胞计数极度低下的患者,临床表现常累及腹部多组织和器官;腹腔淋巴结肿大、肝脾肿大、脂肪肝是其常见的腹部CT表现,还可有腹腔积液、肝脾"镂空"征及肠壁增厚等表现。Objective To explore the clinical characteristics and CT findings of abdomen in patients with AIDS and Penicillium marneffei(PSM).Methods The clinical data and CT images of 186 patients with AIDS and PSM diagnosed in the hospital from January 2008 to June 2019 were retrospectively analyzed.Results All patients had CD4+T lymphocytes less than 50 cells/μL.Abdominal CT was abnormal in 91.39%(170/186)of patients,including 135 cases of abdominal lymphadenopathy(72.58%),110 cases of liver enlargement(59.14%),and spleen 128 cases were enlarged(68.82%),liver and spleen were enlarged in 101 cases(54.30%),fatty liver was in 85 cases(45.70%),25 cases of peritoneal effusion(13.44%),and 19 cases(10.22%)of liver and spleen"hollow out",7 cases(3.76%)thickened the intestinal wall.Conclusion AIDS combined with PSM mostly occurs in patients with extremely low CD4+T lymphocyte counts,and clinical manifestations often involve multiple abdominal tissues and organs.Abdominal lymphadenopathy,hepatosplenomegaly,and fatty liver are common CT findings of the abdomen.The manifestations of peritoneal effusion,liver and spleen"hollow out"signs,and thickening of the intestinal wall.
