踝部神经阻滞麻醉用于糖尿病足清创过程中的麻醉效果观察

摘　　要：目的研究踝部神经阻滞麻醉用于糖尿病足清创过程中的麻醉效果。方法抽选该院收治的82例糖尿病足患者作为研究对象,按入院时间分为两组,2018年4-10月的41例患者为常规组,采用全身麻醉方式;2018年11月-2019年5月的41例患者则为对比组,采用踝部神经阻滞麻醉方式,对比两组患者术后24 h内疼痛数字模拟评分(NRS),以及手术前、麻醉起效后、手术开始时、手术结束前5min的收缩压、舒张压和心率情况。结果手术前,两组患者的NRS评分、收缩压、舒张压和心率差异无统计学意义(P>0.05),手术后,对比组手术后12 h、手术后24 h的NRS评分低于常规组,手术开始时、手术结束前5 min的收缩压、舒张压和心率低于常规组,差异有统计学意义(P<0.05)。结论将踝部神经阻滞麻醉应用到糖尿病足患者手术清创中能有效降低患者的疼痛,使其身体体征更加稳定。Objective To study the anesthesia effect of ankle nerve block anesthesia in diabetic foot debridement.Methods Eighty-two diabetic foot patients admitted in the hospital were selected as the research object,and they were divided into two groups according to the admission time.Forty-one patients from April to October 2018 were the routine group,using general anesthesia.Forty-one patients up to from November 2018 to May 2019 were in the comparison group.Ankle nerve block anesthesia was used to compare the digital analogue score(NRS)of pain within 24 hours after surgery.Systolic blood pressure,diastolic blood pressure and heart rate at the beginning and 5 minutes before the end of the operation.Results Before surgery,the NRS score,systolic blood pressure,diastolic blood pressure,and heart rate of the two groups of patients were not statistically different(P>0.05).After surgery,the NRS scores of the comparison group at 12 h and 24 h after the operation were lower than those of the conventional group.The systolic blood pressure,diastolic blood pressure,and heart rate at 5 min and 5 min before the end of the surgery were lower than those in the conventional group,and the differences were statistically significant(P<0.05).Conclusion Applying ankle nerve block anesthesia to surgical debridement of patients with diabetic foot can effectively reduce the pain of patients and make their physical signs more stable.
