胃萎Ⅰ号方联合穴位贴敷治疗脾胃虚寒型慢性萎缩性胃炎癌前病变42例

摘　　要：目的:观察胃萎Ⅰ号方联合穴位贴敷治疗脾胃虚寒型慢性萎缩性胃炎(chronic atrophic gastritis,CAG)癌前病变的临床疗效。方法:将84例CAG癌前病变患者按照随机数字表法分为对照组和治疗组,每组各42例。对照组给予胃复春片口服治疗,治疗组给予胃萎Ⅰ号方联合穴位贴敷治疗。比较两组患者的临床疗效及治疗前后临床症状积分、胃镜黏膜病理积分变化情况,检测两组患者治疗前后肠三叶因子3(trefoil factor3,TFF3)、核转录因子-κB(nuclear transcription Factor-κB,NF-κB)水平。结果:治疗组有效率为88.10%,对照组有效率为73.81%,两组患者有效率比较,差异具有统计学意义(P<0.05)。两组患者治疗后中医证候积分、TFF3、NF-κB水平及胃黏膜病理积分低于本组治疗前,且治疗组治疗后低于对照组治疗后,差异具有统计学意义(P<0.05)。两组患者治疗后各项症状积分低于本组治疗前,治疗组治疗后胃痛、纳呆、大便稀溏、疲倦乏力、嗳气、气短懒言等症状积分低于对照组治疗后,差异具有统计学意义(P<0.05)。治疗组治疗后胃脘胀满积分与对照组治疗后比较,差异无统计学意义(P>0.05)。结论:胃萎Ⅰ号方联合穴位贴敷治疗脾胃虚寒型CAG癌前病变,可改善患者的临床症状和胃黏膜病理状况,作用机制可能与降低TFF3及NF-κB水平等有关。Objective:To observe the clinical curative effect of Formula Ⅰ for CAG in combination with acupoint application on precancerous lesions of chronic atrophic gastritis(CAG) with deficiency cold syndrome of spleen and stomach.Methods:A total of 84 CAG patients were divided into the control group and the treatment group according to the random number table method,with 42 cases in each group.The control group was treated with Stomach-Recovering Decoction for oral administration,while the treatment group was treated with Formula Ⅰ for CAG in combination with acupoint application.The clinical curative effects and the changes of the scores of the clinical symptoms and the mucosal pathology before and after treatment were compared between the two groups.The levels of trefoil factor 3(TFF3) and nuclear transcription factor-κB(NF-κB) were measured and analyzed.Results:The effective rate was 88.10% in the treatment group and 73.81% in the control group,and the difference was statistically significant(P<0.05).After the treatment,the TCM syndrome score,TFF3,NF-κB levels of each group were lower than those of the same group before the treatment,and all the differences were statistically significant(P<0.05).After the treatment,the scores of symptoms in the two groups were lower than those before the treatment.The scores of symptoms in the treatment group such as stomachache,anorexia,loose stool,tiredness,belching,shortness of breath and laziness to speak were lower than those of the control group,and all the differences were statistically significant(P<0.05).However,there was no significant difference in the score of epigastric fullness between the treatment group and the control group(P>0.05).Conclusion:In terms of treating precancerous lesions of chronic atrophic gastritis(CAG) with deficiency cold syndrome of spleen and stomach,Formula Ⅰ for CAG in combination with acupoint application can improve the clinical symptoms and pathological condition of gastric mucosa,and the mechanism may be related to the decrease of TF
