摘  要:目的探讨钆塞酸二钠(Gd-EOB-DTPA)对肝右叶局灶性病变及肝右叶体素内不相干运动(IVIM)双指数模型及拉伸指数模型测量的影响。资料与方法前瞻性纳入21例肝右叶病变患者22个病灶,分别于Gd-EOB-DTPA注射前后行相同肝脏IVIM序列扫描。分别测量肝右叶实质及右叶病变表观扩散系数(ADC)值、双阶单指数模型真扩散系数(Dmono)、假扩散系数(D^*mono)、灌注分数(fmono)值、双阶双指数模型真扩散系数(Dbi)、假扩散系数(D^*bi)、灌注分数(fbi)值、拉伸指数模型分布扩散系数(DDC)、拉伸指数(α)值;比较增强扫描前后各测量值的差异。结果增强扫描前后肝右叶局灶性病变ADC、Dmono、D~*mono、fmono、Dbi、D^*bi、DDC、α值及肝右叶实质ADC、Dmono、D^*mono、Dbi、D^*bi、α值差异无统计学意义(P均>0.05)。增强扫描后肝右叶局灶病变fbi值和肝右叶实质fmono、fbi值升高,肝右叶实质DDC值降低,差异有统计学意义(t=-2.673、-5.055、-3.722、-2.447,P<0.05)。结论注射Gd-EOB-DTPA对肝右叶局灶病变及肝右叶实质的ADC值、肝右叶病变IVIM参数中双阶单指数及双阶双指数的D值和D^*值、fmono值、拉伸指数模型DDC值及α值定量评估无明显影响,对肝右叶实质背景fmono、fbi、DDC值的定量评估有一定的影响。Purpose To investigate the effect of gadolinium ethoxybenzyl diethylenetriamine pentaacetic acid(Gd-EOB-DTPA)on the measurement of focal lesions and parenchyma in the right liver with the double index model and stretch index model of intravoxel incoherent motion(IVIM).Materials and Methods Twenty-one patients(22 lesions)with focal lesions in right liver were prospectively included in the study.Before and after the injection of Gd-EOB-DTPA,the same sequence of liver IVIM was scanned.Apparent diffusion coefficient(ADC),slow ADC mono(Dmono),fast ADC mono(D*mono),fraction of fast ADC mono(fmono),slow ADC bi(Dbi),fast ADC bi(D*bi),fraction of fast ADC bi(fbi),distributed diffusion coefficient(DDC)and the heterogeneity of intravoxel diffusion(α)of stretch index model of parenchyma and lesions in right liver were measured and calculated respectively.The difference in indexes before and after enhanced scanning was statistically analyzed.Results There was no significant difference in the values of ADC,Dmono,D*mono,fmono,Dbi,D*bi,DDC andαof lesions in right liver and the values of ADC,Dmono,D*mono,Dbi,D*bi andαof right liver parenchyma between those two groups before and after enhancement(P all>0.05).After enhancement,fbi of lesions in right liver,fmono and fbi of the right liver parenchyma increased,DDC values decreased,and there was significant difference before and after enhancement(t=-2.673,-5.055,-3.722,-2.447,P<0.05).Conclusion Gd-EOB-DTPA has no significant influence in quantitative assessment with ADC of lesions and parenchyma in right liver,and with D and D*value of single and double index,fmono,DDC andαvalue of stretching index model among IVIM parameters of lesions in right liver.Gd-EOB-DTPA may have influence on quantitative assessment with fmono,fbi and DDC of right liver parenchyma.
