扁桃体单极电刀切除术与剥离术的自身对照研究

摘　　要：目的 比较扁桃体单极电刀除切术与扁桃体剥离术的优缺点.方法 62例需要行双侧扁桃体切除术的患者,随机确定一侧扁桃体入实验组,行单极电刀切除术;对侧扁桃体入对照组,行扁桃体剥离术.观察及对比两组术中出血量、手术持续时间、术后1~7 d疼痛程度、术后原发性出血及继发性出血的发生率、白膜脱落时间.结果 实验组术中出血量、手术持续时间均少于对照组(P<0.01).实验组术后第1、2天的疼痛程度评分低于对照组(P<0.01);术后第4、5天则高于对照组(P<0.01),术后白膜脱落时间也长于对照组(P<0.01).结论 扁桃体单极电刀切除术虽然能减少手术时间和术中出血量,但术后疼痛时间和白膜脱落时间显著延长,故单极电刀切除扁桃体并不全面优于扁桃体剥离术.从临床安全角度考虑,扁桃体剥离术更有优势.Objective To compare the advantages and disadvantages of unipolar electrotome tonsillectomy and traditional tonsillectomy. Methods A total of 62 patients who underwent bilateral tonsillectomy were enrolled in the study. The tonsils of enrolled patients on the one side were resected by unipolar electrotome and included in the Experimental Group while those on the other side were resected by traditional tonsillectomy and included in the Control Group. The intraoperative blood loss, the duration of operation, the degree of pain at postoperative 1-7 d, the incidence of primary and secondary postoperative bleeding, and the shedding time of tunica albuginea of the two groups were observed and compared. Results The Experimental Group had the intraoperative blood loss and the duration of operation less than the Control Group(P<0.01). The Experimental Group had the pain rating lower than the Control Group at postoperative 1 and 2 d(P<0.01), but higher than the Control Group at postoperative 4 and 5 d(P<0.01). The Experimental Group had the shedding time of tunica albuginea longer than the Control Group(P<0.01). Conclusion Unipolar electrotome tonsillectomy can reduce the operation duration and intraoperative blood loss, but significantly prolongs the time of postoperative pain and the shedding time of tunica albuginea. Therefore, unipolar electrotome tonsillectomy is incompletely superior to traditional tonsillectomy. From the perspective of clinical safety, tonsillectomy has more advantages.
