改良经皮微创缝合治疗急性闭合性跟腱断裂的效果研究

摘　　要：目的:通过比较改良经皮微创缝合治疗和内侧切开开放治疗急性跟腱断裂的临床效果和并发症发生率,评估改良经皮微创缝合治疗的临床疗效。方法:收集广西医科大学第一附属医院2016年1月至2018年1月收治的50例急性跟腱断裂患者,按手术方式分为两组,内侧切开组(22例)和改良经皮微创缝合组(28例)。所有患者均遵循相同的康复方案。收集患者手术时间、切口长度、住院时间、并发症发生率、美国矫形踝足协会评分标准(AOFAS)和跟腱完全断裂评分标准(ATRS)评分。结果:两组患者在AOFAS和ATRS评分、并发症发生率间比较,差异无统计学意义(P>0.05)。改良经皮微创缝合组手术时间[(32.18±5.56)min]、切口长度[(3.37±6.26)cm]和住院时间[(6.86±2.14)d]均明显短于内侧切开组[(66.36±9.42)min、(9.85±4.47)cm、(9.04±3.83)d](均P<0.05)。结论:改良经皮微创缝合是一种安全有效的治疗方法,可为急性跟腱断裂患者提供良好的术后功能结果和较低的并发症发生率。Objective:To compare the clinical effect and the incidence of complications between modified percutaneous minimally invasive suture andopen medial incision in the treatment of acute Achilles tendon rupture,and to evaluate the clinical efficacy of modified percutaneous minimally invasive suture treatment.Methods:50 patients with acute Achilles tendon rupture admitted to the First Affiliated Hospital of Guangxi Medical University from January 2016 to January 2018 were collected and divided into two groups according to the surgical method,which were the medial incision group(n=22)and the modified percutaneous minimally invasive suture group(n=28).All patients werefollowed the same rehabilitation program.The data of operation time,incision length,hospitalization time,complication rate,American Orthopaedic Foot&Ankle Society(AOFAS)score,and Achilles Tendon Total Rupture Score(ATRS)were collected.Results:There was no significant difference between the two groups of patients in the AOFASscore,ATRS score,and the incidence of complications(P>0.05).The operation time(32.18±5.56 min),incision length(3.37±6.26 cm)and hospital stay(6.86±2.14 d)of the modified percutaneous minimally invasive suture group were significantly shorter than those of the medial incision group(66.36±9.42 min);(9.85±4.47)cm;and(9.04±3.83 d)(all P<0.05).Conclusion:Modified percutaneous minimally invasive suture is a safe and effective treatment method,which can provide good postoperative functional results and lower incidence of complications for patients with acute Achilles tendon rupture.
