健脾脱敏清络汤结合常规治疗对肾病综合征型紫癜性肾炎的影响

摘　　要：目的:探讨健脾脱敏清络汤结合常规治疗对肾病综合征型紫癜性肾炎(NSPN)的影响。方法:选取沧州中西结合医院2016年5月至2019年5月收治的NSPN患者102例。按随机数字表法分成西药组和中药组,每组51例。西药组给予常规治疗,中药组采用健脾脱敏清络汤治疗。两组治疗4周为1疗程,均治疗3个疗程。比较两组临床疗效、治疗前、后中医证候症状评分的变化以及血清免疫球蛋白E(IgE)、白介素-16(IL-16)、白介素-18(IL-18)水平。经Pearson线性相关分析血清IgE、IL-16及IL-18与中医证候评分的相关性,观察两组不良反应率。结果:中药组总有效率为92.16%,较西药组的76.47%显著增高(P<0.05)。两组治疗后中医证候积分和血清IgE、IL-16、IL-18显著低于治疗前,且中药组较西药组显著降低(P<0.05)。Pearson分析提示血清IgE、IL-16、IL-18与中医证候积分呈正相关关系(r=0.573、0.614、0.609,P<0.05)。两组不良反应率比较,差异无统计学意义(P>0.05)。结论:与常规治疗相比,NSPN患者经健脾脱敏清络汤治疗,能进一步改善中医症状,下调血清IgE、IL-16、IL-18水平,且未引起严重不良反应,总体疗效理想。Objective:To explore the effect of Jianpi Tuomin Qingluo decoction in combination with routine treatment on nephrotic syndrome purpura nephritis(NSPN).Methods:102 patients with NSPN from May 2016 to May 2019 were selected from our hospital.According to random number table,the patients were divided into western medicine group and traditional Chinese medicine group,with 51 cases in each group.The western medicine group was given routine treatment,and the traditional Chinese medicine group was treated with Jianpi Tuomin Qingluo decoction.The two groups were treated for 4 weeks as a cycle of treatment,and both groups were treated for 3 cycles.The clinical efficacy,the changes of TCM syndrome scores before and after treatment,and the levels of serum immunoglobulin E(IgE),interleukin-16(IL-16)and interleukin-18(IL-18)were compared between the two groups.Pearson linear correlation analysis was used to analyze the correlation between serum IgE,IL-16,IL-18 and TCM syndrome score,and the adverse reaction rate of the two groups was observed.Results:The total effective rate of the traditional Chinese medicine group was 92.16%,which was significantly higher than that of the western medicine group(76.47%)(P<0.05).The scores of TCM syndrome and serum level of IgE,IL-16 and IL-18 in the two groups were significantly lower after treatment than those before treatment,especially in traditional Chinese medicine group(P<0.05).Pearson analysis showed that there was a positive correlation between serum IgE,IL-16,IL-18 and TCM syndrome score(r=0.573,0.614,0.609 P<0.05).There was no significant difference in the adverse reactions rate between the two groups(P>0.05).Conclusion:Compared with conventional therapy,the treatment of NSPN patients with Jianpi Tuomin Qingluo decoction can further improve the symptoms of traditional Chinese medicine,down-regulate the levels of serum IgE,IL-16 and IL-18,and has no serious adverse reactions.The overall efficacy is ideal.
