摘　　要：目的观察炎症性肠病(IBD)患儿血浆25-羟维生素D[25(OH)VitD]、铁调素(Hepc)、血红蛋白(Hb)的表达变化,并探讨其临床意义。方法选取IBD患儿187例,其中溃疡性结肠炎(UC组)131例,克罗恩病(CD组)56例,并以40例健康儿童作为对照组,应用酶联免疫吸附实验(ELISA)法检测血浆中25(OH)VitD、Hepc、Hb水平,统计学分析组间各指标的表达差异及各指标间相关性。结果与对照组相比,UC组及CD组血浆25(OH)VitD、Hb水平降低,而血浆Hepc水平升高(P均<0.05)。UC组及CD组血浆25(OH)VitD、Hepc、Hb水平与患儿性别、年龄、疾病类型、病变范围无关(P均>0.05)。与缓解期UC及CD患儿相比,活动期UC及CD患儿血清25(OH)VitD、Hb较低,而Hepc较高(P均<0.05)。UC组及CD组血浆25(OH)VitD水平与Hepc呈负相关(r分别为-0.634、-0.620,P均<0.05),与Hb呈正相关(r分别为0.611、0.589,P均<0.05)。结论IBD患儿血浆25(OH)VitD、Hb水平降低,而Hepc水平升高,三者与IBD疾病活动性有关;检测血浆25(OH)D、Hepc、Hb水平有助于IBD患儿病情判断。Objective To observe the expression changes of plasma 25-hydroxyvitamin D[25(OH)VitD],hepcidin(Hepc)and hemoglobin(Hb)in children with inflammatory bowel disease(IBD)and to explore their clinical significance.Methods Totally 187 children with IBD were selected,including 131 cases of ulcerative colitis(UC group)and 56 cases of Crohn's disease(CD group).Forty healthy children were taken as the control group.The levels of plasma 25(OH)VitD,Hepc,and Hb were detected by enzyme-linked immunosorbent assay(ELISA).The expression differences and correlation between the indexes were analyzed statistically.Results Compared with the control group,the levels of plasma 25(OH)VitD and Hb in the UC group and CD group decreased,while the level of plasma Hepc increased(all P<0.05).The levels of plasma 25(OH)VitD,Hepc and Hb in the UC group and CD group were not related to the sex,age,disease type or lesion range(all P>0.05).Compared with children in remission of UC and CD,children with active UC and CD had lower levels of plasma 25(OH)VitD and Hb,while had higher Hepc level(all P<0.05).The level of plasma 25(OH)VitD was negatively correlated with Hepc(r=-0.634,-0.620,both P<0.05),and was positively correlated with Hb(r=0.611,0.589,both P<0.05).Conclusions Plasma 25(OH)VitD and Hb levels decrease in children with IBD,while Hepc level increases.These three indexes are related to IBD disease activity.The detection of plasma 25(OH)D,Hepc,and Hb is helpful to the diagnosis of IBD patients.
