PDCA循环质控管理措施在三升袋配置中的应用

摘　　要：目的探究戴明环(PDCA)循环质控管理措施在三升袋配置中的应用效果,为患者临床用药安全提供参考。方法回顾性分析2018年3月至2019年3月间肇庆市第二人民医院药剂科配置的三升袋1 630例临床配置情况,对2018年3~9月间采取常规管理措施的815例(管理前)以及2018年10月至2019年3月间采取PDCA循环质控管理措施的815例进行分析(管理后),比较采用PDCA循环质控管理措施前后未严格执行无菌操或未使用洁净台面,配置人员未穿着洁净工作服或隔离衣、洗手以及酒精擦拭未至肘部以上,未现配现用,未标明床号、姓名、配置时间以及成分,不能立即使用的未避光保存在4℃冰箱内等问题出现情况以及患者使用三升袋后不良反应发生情况。结果管理后,未严格执行无菌操作或未使用洁净台面,配置人员未穿着洁净工作服或隔离衣、洗手以及酒精擦拭未至肘部以上,未现配现用,未标明床号、姓名、配置时间以及成分,不能立即使用的未避光保存在4℃冰箱内等配置过程中存在的问题与管理前比较明显减少,差异均有统计学意义(P<0.05);管理后,出现寒颤以及发热症状,出现胸痛、呼吸困难以及胸闷症状,出现恶心症状,出现皮疹症状的患者与管理前比较明显减少,差异均有统计学意义(P<0.05)。结论 PDCA循环质控管理措施有助于规范护士在三升袋配置中的各项操作,各项操作的规范管理保证了患者用药安全,能够有效降低患者在使用过程中不良反应发生情况。Objective To investigate the application effect of Dai Minghuan(PDCA)cycle quality control management measures in preparation of three-liter bag,and to provide reference for the safety of clinical drug use.Methods A retrospective analysis was made on 1630 cases of clinical configuration of three-liter bags in Department of Pharmacy,the Second People's Hospital of Zhaoqing from March 2018 to March 2019,including 815 cases taking routine management measures from March to September,2018 and 815 cases taking PDCA circular quality control management measures between October 2018 and March 2019.Before and after PDCA circular quality control management measures were adopted,the following problems during preparation were compared:aseptic operation was not strictly carried out or clean table was not used;personnel were not wearing clean work clothes or isolation clothes;hand washing and alcohol wiping were not above elbow;the bags was not used immediately after preparation;bed number,name,and preparation time,and ingredients were not marked,the bags that can not be used immediately are not stored in 4℃refrigerator without light.The adverse reactions of patients after using three-liter bags were also compared.Results After the management,the problems during preparation were significantly reduced compared with before management(P<0.05).After the management,the number of patients with chills and fever symptoms,chest pain,dyspnea,chest tightness,nausea,and rash symptoms was significantly reduced compared with those before management(P<0.05).Conclusions The PDCA cycle quality control management measures help to standardize the various operations of nurses in the preparation of three-liter bag,which ensures the patient's medication safety and can effectively reduce the occurrence of adverse reactions.
