右美托咪定复合舒芬太尼清醒镇静术在腰腹部微创吸脂手术中的应用

摘　　要：目的探究右美托咪定复合舒芬太尼清醒镇静术对腰腹部微创吸脂手术患者的应用效果。方法选取我院2018年10月至2020年4月收治的200例腰腹部微创吸脂手术患者作为研究对象,采用随机数字表法将这些患者随机均分为研究组与对照组,对照组采用舒芬太尼+丙泊酚诱导、丙泊酚持续麻醉的传统方案,而研究组患者采用舒芬太尼+右美托咪定诱导、右美托咪定持续麻醉的方案。记录两组患者在术前5min时(T1)、麻醉后5min(T2)、肿胀液注射时(T3)、手术结束时(T4)平均动脉压(MAP)、心率(HR)等指标,同时在以上时间点对患者体内血糖(GLU)水平进行测定。并记录患者手术过程及患者术后的不良反应。结果两组患者术后均有一定例数的患者发生不良反应,但研究组患者更少(P<0.05)。与T1相比,两组患者T2、T3、T4时的MAP、HR、GLU均不同程度的升高,尤其是在T2时升高更明显。研究组与对照组相比,术前的MAP、HR、GLU均无差异(P>0.05);而T2、T3、T4时,研究组的MAP、HR、GLU均较对照组更低(P<0.05)。结论腰腹部微创吸脂手术患者采用右美托咪定复合舒芬太尼的麻醉方案,患者一般状况更为稳定,术中应激反应程度更低,不良反应更少。Objective To investigate the effect of dexmedetomidine combined with sufentanil sober sedation on patients with minimally invasive liposuction surgery.Methods Select 200 patients with minimally invasive liposuction surgery in our hospital from October 2018 to April 2020 as the research object.Random number table method was used to randomly divide these patients into a research group and a control group.The traditional regimen of fentanyl+propofol induction and continuous propofol anesthesia,while the patients in the study group used sufentanil+dexmedetomidine induction and dexmedetomidine continuous anesthesia.Record the average arterial pressure(MAP)and heart rate(HR)of the two groups of patients at 5 minutes before operation(T1),5 minutes after anesthesia(T2),at the time of swelling fluid injection(T3),at the end of operation(T4),and at the same time The blood glucose(GLU)level in patients was measured at the above time points.And record the patient's operation process and the patient's postoperative adverse reactions..Results A certain number of patients had adverse reactions after surgery in both groups,but there were fewer patients in the study group(P<0.05).Compared with T1,MAP,HR,and GLU of T2,T3,and T4 in two groups of patients increased to varying degrees,especially at T2.Compared with the control group,the study group had no difference in MAP,HR,and GLU before surgery(P>0.05);while at T2,T3,and T4,the study group had lower MAP,HR,and GLU than the control group(P<0.05).Conclusion Patients with minimally invasive liposuction of the waist and abdomen are treated with dexmedetomidine combined with sufentanil.The general condition of the patient is more stable,the degree of stress response during the operation is lower,and the adverse reactions are less.
