点阵式CO2激光治疗外阴硬化性苔癣的临床疗效研究

摘　　要：目的探讨点阵式CO2激光治疗外阴硬化性苔癣的临床疗效研究。方法将2018年10月-2019年10月在我院皮肤科治疗的76例外阴硬化性苔癣患者随机分为两组,对照组实施曲安奈德益康唑乳膏治疗,观察组实施点阵式CO2激光治疗,对比两组的皮肤症状VAS评分变化、不良反应、女性性苦恼量表(FSDS-R)及皮肤病生活质量指数(DLQI)变化。结果观察组第1次治疗后1月、总疗程结束后1月、总疗程结束后3月的外阴瘙痒、皮肤干燥、性交痛等VAS评分明显低于对照组(P<0.05);观察组感染、溃破、过敏、外阴灼烧感等不良反应发生率与对照组相比无明显差异(P>0.05);观察组第1次治疗后1月、总疗程结束后1月、总疗程结束后3月的FSDS-R评分明显低于对照组(P<0.05),但两组DLQI评分无明显差异(P>0.05)。结论点阵式CO2激光治疗外阴硬化性苔癣的临床疗效显著,对外阴症状改善明显,有效降低了女性苦恼,提高了生活质量,且不良反应发生率低,具有较好的疗效及安全性。Objective to investigate the clinical efficacy of dot-matrix CO2 laser in the treatment of vulvar sclerosing lichen.Methods In October 2018-October 2019 in our hospital dermatology treatment of 76 cases of vulvar sclerosing lichen patients were randomly divided into two groups:control group in implementation of triamcinolone acetonide econazole cream treatment,observation group implement dot-matrix CO2 laser treatment,compared two groups of skin symptoms VAS score changes,adverse reactions,female sex distress scale(FSDS-R)and dermatology life quality index(DLQI)change.Results VAS scores of vulva itching,skin dryness and sexual pain in the observation group were significantly lower than those in the control group(P<0.05).There was no significant difference between the observation group and the control group in the incidence of infection,ulceration,allergy,vulvar burning sensation and other adverse reactions(P>0.05).Fsds-r score of the observation group was significantly lower than that of the control group(P<0.05)1 month after the first treatment,1 month after the end of the total course of treatment,and 3 months after the end of the total course of treatment,but there was no significant difference in DLQI score between the two groups(P>0.05).Conclusion dot-matrix CO2 laser treatment of vulvar sclerosing lichen has a significant clinical effect,significantly improves vulvar symptoms,effectively reduces female distress,improves quality of life,and has a low incidence of adverse reactions,with good efficacy and safety.
