妊娠期高血压疾病患者心脏Tei指数与子宫动脉频谱相关性研究及临床应用

摘　　要：目的探讨妊娠期高血压疾病患者心脏Tei指数与子宫动脉频谱的相关性及临床应用价值。方法回顾性分析2016年1月~2020年1月在本院诊治的169例妊娠期高血压疾病孕妇及100例正常孕妇的临床资料。妊娠期高血压疾病孕妇根据其疾病严重程度的不同进行分组,将子痫前期89例孕妇的临床资料纳入A组,将重度子痫前期80例孕妇的临床资料纳入B组,将血压正常的100例正常孕妇的临床资料纳入C组。三组孕妇均进行超声心动图检测,并计算左右心室的Tei指数及子宫动脉频谱[子宫动脉多普勒收缩期与舒张期流速比(S/D)],分析心脏Tei指数与S/D的相关性。结果检测后,A组、B组的Tei指数及S/D指标均高于C组,且B组高于A组,差异有统计学意义(F=2550.95、3577.35,P=0.000、0.000);经Pearson分析,娠期高血压患者Tei指数与S/D指标之间成正相关(r=0.778,P=0.000)。结论妊娠期高血压疾病越严重,心脏Tei指数与S/D指标越高,且成正相关性。Objective To investigate the correlation between heart Tei index and uterine arterial spectrum in patients with gestational hypertension and its clinical application value.Methods The clinical data of 169 pregnant women with gestational hypertension and 100 normal pregnant women who were diagnosed and treated in the hospital from January 2016 to January 2020 were retrospectively analyzed.The pregnant women with gestational hypertension were divided into groups according to the severity of the disease.The clinical data of 89 pregnant women with preeclampsia were included in group A,the clinical data of 80 pregnant women with severe preeclampsia were included in group B,and the clinical data of 100 normal pregnant women with normal blood pressure were included in group C.The three groups of pregnant women underwent ultrasonic cardiogram examination,and the Tei index of left and right ventricles and uterine arterial spectrum(uterine artery Doppler ratio of systolic to diastolic velocity[S/D])were calculated.The correlation between heart Tei index and S/D was analyzed.Results After detection,the Tei index and S/D in group A and group B were higher than those in group C,and which in group B was higher than that in group A,and the differences were statistically significant(F=2550.95,3577.35,P=0.000,0.000).Pearson analysis showed that Tei index was positively correlated with S/D in patients with gestational hypertension(r=0.778,P=0.000).Conclusion The more severe the gestational hypertension,the higher the heart Tei index and the S/D index,and Tei index is positively correlated with S/D.
