摘  要:目的探讨原发性高血压合并冠心病患者中ACE I/D基因多态性与胱抑素C(Cys-C)的相关性.方法选取2018年1月至2019年7月我院患者共计282例,其中高血压合并冠心病组172例,单纯原发性高血压组110例,采用实时荧光定量PCR检测ACE I/D基因多态性,同时检测Cys-C及血尿相关生化指标.结果(1)原发性高血压合并冠心病组的ACE I/D基因中DD基因型及D等位基因频率高于单纯原发性高血压组(χ2=11.186,P<0.05;χ2=9.193,P<0.05);(2)原发性高血压合并冠心病组DD基因型患者的Cys-C、尿微量白蛋白、24 h尿微量白蛋白排泄率水平较ID型、II型更高(P<0.05);(3)原发性高血压合并冠心病的DD基因型在Cys-C>1.03的分布频率更高(χ2=8.47,P<0.05),且有无D等位基因时对应的Cys-C值相关的ROC曲线提示曲线下面积为0.662(P<0.05).结论原发性高血压合并冠心病患者ACE I/D基因多态性与Cys-C有明显相关性,DD基因型患者有着更高的Cys-C水平.Objective To investigate the relationship between the polymorphism of ACE I/D gene and Cys⁃C in patients with essential hypertension combined with coro⁃nary heart disease.Methods A total of 282 inpatients were selected from our hospital from January 2018 to July 2019.There were 172 cases of essential hypertension com⁃bined with coronary heart disease and 110 cases of simple essential hypertension.Us⁃ing real⁃time quantitative PCR to detect the polymorphism of ACE I/D gene,Cys⁃C and biochemical index were tested else.Results(1)The frequency of DD genotype and D allele in ACE gene of essential hypertension combined with coronary heart dis⁃ease were higher than that of primary essential hypertension(P<0.05).(2)The level of Cys⁃C,UMA and 24HUAE in patients with DD genotype of essential hyper tension c⁃ombined with coronary heart disease was higher than that of ID genotype and II geno⁃type(P<0.05).(3)DD genotypes were more frequently distributed in the Cys⁃C>1.03 mg/L group in patients with essential hypertension combined with coronary heart dis⁃ease(χ2=8.47,P<0.05),the ROC curve of the corresponding Cys⁃C value with or without the D allele suggested that the area under the curve was 0.652(P<0.05).Conclusion There is a significant correlation between ACE I/D gene polymorphism and Cys⁃C in patients with essential hyperte⁃nsion combined with coronary heart disease,Patients with DD genotype have a higher level of Cys⁃C.
