摘　　要：目的:探讨唑来膦酸联合鲑鱼降钙素治疗骨质疏松性踝关节骨折术后的临床疗效。方法:选取我科2017年1月—2018年12月收治的踝关节骨折患者48例,分为观察组和对照组,两组患者均接受踝关节骨折切开复位内固定手术,术后均给予钙剂及唑来膦酸钠治疗,观察组在上述基础上再给予鲑鱼降钙素治疗。比较两组患者术前及术后骨密度情况、骨转换标志物水平、VAS评分、踝关节功能恢复情况和术后并发症发生状况等。结果:与对照组比较,术后观察组骨密度值更高,血清PINP和β-CTX水平均降低更为明显。术后1周、1个月、3个月,观察组VAS评分高于对照组,差异有统计学意义(P<0.05),而术前、术后6个月、12个月,两组患者VAS评分差异无统计学意义(P>0.05)。术后3个月,观察组Maryland足功能评分低于对照组,差异有统计学意义(P<0.05)。而术后6个月、12个月,两组患者Maryland足功能评分差异无统计学意义(P>0.05)。结论:唑来膦酸联合鲑鱼降钙素治疗骨质疏松性踝关节骨折术后的临床疗效较好,值得在临床上推广应用。Objective:To explore the Clinical effect of zoledronic acid combined with salmon calcitonin on postoperative osteoporotic ankle fracture.Methods:48 patients with ankle fracture from January 2017 to December 2018 in our department were divided into the observation group and the control group.Both groups received open reduction and internal fixation of ankle fracture.Both groups were treated with calcium and sodium zoledronate after operation.The observation group was treated with salmon calcitonin on the above basis.Bone density,bone turnover marker level,VAS score,ankle function recovery and postoperative complications of the two groups were compared.Results:Compared with the control group,the bone mineral density of the postoperative observation group was higher,and the serum PINP andβ-CTX levels were decreased more significantly.The VAS score of the observation group was higher than that of the control group at 1 week,1 month and 3 months after operation,and the difference was statistically significant(P<0.05).Whereas there was no significant difference in VAS scores at 6 and 12 months after operation(P>0.05).The Maryland score of the observation group was lower than that of the control group in 3 months after operation,and the difference was statistically significant(P<0.05).Whereas there was no significant difference in Maryland score at 6 and 12 months after operation(P>0.05).Conclusion:Zoledronic acid combined with salmon calcitonin in the treatment of osteoporotic ankle fracture has a good clinical effect,which is worthy of clinical application.
