摘　　要：目的分析PauwelsⅢ型股骨颈骨折患者并发股骨头缺血性坏死的危险因素及护理干预效果。方法回顾性分析2016年6月至2019年6月我院就诊的120例PauwelsⅢ型股骨颈骨折患者的临床资料,根据是否并发股骨头缺血性坏死分为坏死组21例和未坏死组99例。分析PauwelsⅢ型股骨颈骨折患者并发股骨头缺血性坏死的危险因素。结果经单因素分析,年龄、手术方式、术前牵引、复位质量、骨折移位、负重活动与PauwelsⅢ型股骨颈骨折患者并发股骨头缺血性坏死相关(P<0.05)。经多因素Logistic回归分析,年龄、手术方式、术前牵引、复位质量、骨折移位、负重活动是PauwelsⅢ型股骨颈骨折患者并发股骨头缺血性坏死的独立危险因素(P<0.05)。结论年龄、手术方式、术前牵引、复位质量、骨折移位、负重活动是PauwelsⅢ型股骨颈骨折患者并发股骨头缺血性坏死的危险因素,需采取针对性护理措施,降低股骨头缺血性坏死的发生率。Objective To analyze the risk factors and nursing intervention effect on Pauwels typeⅢfemoral neck fracture complicated with avascular necrosis of femoral head in patients.Methods To retrospectively analyze the clinical data of 120 cases of patients with Pauwels typeⅢfemoral neck fractures in our hospital from June 2016 to June 2019,and they were divided into 21 cases in the necrosis group and 99 cases in the non-necrosis group according to whether or not complicated with avascular necrosis of the femoral head to analyze the risk factors of Pauwels typeⅢfemoral neck fracture complicated with avascular necrosis of the femoral head.Results Univariate analysis showed that the age,surgical method,preoperative traction,quality of reduction,fracture displacement and weight-bearing activities were associated with Pauwels typeⅢfemoral neck fracture complicated with avascular necrosis of the femoral head(P<0.05).Multivariate logistic regression analysis showed that the age,surgical method,preoperative traction,quality of reduction,fracture displacement and weight-bearing activities were the independent risk factors for Pauwels typeⅢfemoral neck fracture complicated with avascular necrosis of the femoral head(P<0.05).Conclusion The age,surgical method,preoperative traction,quality of reduction,fracture displacement and weight-bearing activities were the risk factors for Pauwels typeⅢfemoral neck fracture complicated with avascular necrosis of the femoral head.Medical staff need to take targeted nursing measures to reduce the incidence rate of avascular necrosis of the femoral head.
