腹腔镜手术与开放手术治疗胆总管结石的疗效分析

摘　　要：目的:探讨腹腔镜胆总管探查术(LCBDE)与开放手术治疗胆总管结石的疗效。方法:将70例因胆总管结石行手术治疗的患者根据手术时期不同分为腔镜组(32例)和开放组(38例)。比较两组患者手术时间、手术中出血量及手术后排气时间、住院时间、并发症。结果:两组患者手术时间、手术后并发症方面差异均无统计学意义(P>0.05)。在出血量、排气时间、手术后住院时间方面腔镜组明显优于开放组(P<0.05)。与开放组比较,腔镜组出血量少(P<0.001),腔镜组术后排气早(P<0.001),腔镜组术后住院天数短(P<0.001)。结论:LCBDE治疗胆总管结石安全有效,手术创伤轻,手术后恢复快,值得临床推行。Objective:Effect and application value of laparoscopic common bile duct exploration and open operation in treatment of common bile duct stones were compared.Methods:Seventy patients with common bile duct stones and Gallbladder stones were treated with operation.All the cases were divided into two groups according to the different operation period:laparoscopic common bile duct exploration group and open operation group.The operating time,bleeding volume,postoperative ventilation time,postoperative complications and postoperative hospital stay were compared.Results:There were no statistical differences between the two groups in the operation time and postoperative complications(P>0.05).Compared to the open operation,LCBDE group had shorter postoperative ventilation time,less bleeding volume,and shorter postoperative hospital stay.The differences were statistically significant(P<0.05).Conclusions:LCBDE in treating common bile duct stone is safe and effective with small trauma and quicker recovery.It is worth clinical promotion.
