摘 要:目的:探讨濉溪县丙型肝炎病毒(HCV)的感染情况及基因型的分布特征,以便为慢性丙型肝炎的治疗提供参考。方法:将2017年1月至2018年6月期间在濉溪县医院感染疾病科接受治疗的170例HCV-RNA阳性感染患者作为研究对象。使用ABI7500实时荧光定量PCR仪及ABI-3500基因测序仪对170例患者进行HCV基因型检测,并分析其HCV感染情况与其HCV基因型的相关性。结果:170例患者中丙型肝炎病毒基因1型132例(77.65%),基因2型33例(19.41%),基因3型1例(0.59%),基因4-6型4例(2.35%);其中30例患者进行丙肝病毒基因亚型检测,24例为HCV 1b基因型,1例1a基因型,5例为2a基因型。感染途径主要为有偿献血感染,占67.65%(115/170)。病毒载量(t=0.03 p=0.98)、血清酶学(t=0.14 p=0.89)及疾病进展(X2=0.26 P=0.61)方面,基因1型和基因2型丙型肝炎病毒感染者之间差异无统计学意义(p>0.05)。结论:本地区丙型肝炎病毒基因型主要为基因1型,其次基因2型,基因亚型以基因1b为主。感染途径以有偿献血为主,基因1型较基因2型在病毒载量、酶学及疾病进展无明显差异.Objective:To investigate the distribution and clinical characteristics of hepatitis C virus genotypes in the region.Methods:The genotypes of hepatitis C virus in 170 RNA-positive patients with hepatitis C were detected by ABI 7500 real-time fluorescence quantitative PCR and ABI-3500 gene sequencer,and the correlation between genotypes and clinical manifestations was analyzed.Results:Among 170 patients,132(77.65%)were hepatitis C virus genotype 1,33(19.41%)were genotype 2,1(0.59%)was genotype 3,and 4(2.35%)were genotype 4-6.Among them,30 patients were tested for HCV gene subtype,The genotype 1b,1a and 2a accounted for 24 cases,1 cases and 5 cases respectively.The main route of infection was commercial blood donation,accounting for 67.65%(115/170).There was no significant difference in viral load(t=0.03 P=0.98),serum enzymology(t=0.14 P=0.89)and disease progression(X2=0.26 P=0.61)between genotype 1 and genotype 2 hepatitis C virus infections(p>0.05).Conclusion:Hepatitis C virus genotype is mainly genotype 1,followed by genotype 2,genotype 1b is the main subtype.The main route of infection was commercial blood donation.There was no significant difference in viral load,enzymology and disease progression between genotype 1 and genotype 2.
