Quadrant系统下经椎间孔腰椎椎体间融合术联合椎弓根内固定术治疗腰椎滑脱症的效果  

Effect of transforaminal lumbar interbody fusion combined with pedicle internal fixation through Quadrant system in the treatment of lumbar spondylolisthesis

摘　　要：目的探讨Quadrant系统下经椎间孔腰椎椎体间融合术(TLIF)联合椎弓根内固定术治疗腰椎滑脱症的效果。方法将60例腰椎滑脱症患者采用随机数字表法分为对照组和研究组,各30例。对照组行传统开放手术,研究组行Quadrant系统下TLIF联合椎弓根内固定术。比较两组治疗效果。结果研究组的手术切口小于对照组,术中出血量及术后引流量少于对照组,但手术时长长于对照组(P<0.05)。治疗后1周及1个月,两组的VAS及ODI评分均较治疗前降低,且研究组低于对照组(P<0.05)。研究组的治疗优良率高于对照组(P<0.05)。结论 Quadrant系统下TLIF结合椎弓根内固定术治疗腰椎滑脱症,手术切口小,术中出血少,疼痛较轻,可显著改善腰椎功能。Objective To investigate the effect of transforaminal lumbar interbody fusion(TLIF)combined with pedicle internal fixation through Quadrant system in the treatment of lumbar spondylolisthesis.Methods Sixty patients with lumbar spondylolisthesis were divided into control group and study group by random number table method,with 30 cases in each group.The control group received traditional open surgery,and the study group received TLIF combined with pedicle internal fixation through Quadrant system.The therapeutic effects of the two groups were compared.Results The surgical incision of the study group was smaller than that of the control group,and intraoperative bleeding volume and postoperative drainage volume were less than those of the control group,but the operation duration was longer than that of the control group(P<0.05).One week and one month after the treatment,the VAS and ODI scores of the two groups were lower than those before treatment,and those of the study group were lower than the control group(P<0.05).The excellent and good rate of treatment in the study group was higher than that in the control group(P<0.05).Conclusion The TLIF combined with pedicle internal fixation through Quadrant system in the treatment of lumbar spondylolisthesis has the advantages of small incision,less bleeding and less pain,it can significantly improve lumbar function.
